特型真空垫在宫颈癌调强放疗中的应用 被引量:3

摘要 目的:探讨特型真空垫固定技术在宫颈癌调强放疗中的临床应用价值及存在的问题。方法:采用真空垫盆腔抬高体位固定技术,利用激光线标出真空袋与身体之间的关系,在患者的相对应部位和真空袋作连线标记,以便患者体位重复。在放疗实施前EPID射野验证,并进行比较,通过逆向三维计划系统对小肠、膀胱危急器官的受量评估。结果:(1)根据特型真空垫标记摆位,应用EPID射野验证,X方向的误差为(0.31±0.18)cm,Y方向的误差为(0.38±0.32)cm,Z方向的误差为(0.26±0.22)cm。(2)小肠受量比较:通过逆向计划系统对小肠、膀胱危急器官的评估,该技术能使小肠的受照射体积减少56%,直肠受照射体积减少45%,膀胱的受照射体积减少32%,PTV体积相同,急性反应明显减少。结论:在宫颈癌调强放疗过程中,用特型真空垫盆腔抬高固定体位的方法能明显减少摆位偏差,减少危及器官照射体积,降低危急器官急性反应,具有一定的临床应用价值。
