剖宫产妇女放置三种IUD的临床效果比较

The Clinical Study of Three Types of IUD in Women with History of Cesarean Section

目的:观察剖宫产术后妇女放置吉妮致美(GeneFiexIN)IUD、元宫IUD、TCu220c IUD的临床效果。方法:对380例有剖宫产史的受术者,随机放置吉妮致美、元宫及TCu220c,随访24个月后,观察效果。结果:随访24个月后,吉妮致美IUD带器妊娠率为0.85%,脱落率为0.85%,因症取出率为1.69%,续用率为96.67%;元宫IUD带器妊娠率为5.79%,脱落率为6.61%,因症取出率为5.65%,续用率为83.08%;TCu220c IUD带器妊娠率为5.83%,脱落率为6.56%,因症取出率为6.67%,续用率为82.31%。结论:放置吉妮致美IUD具有不良反应小,脱落率低等优点,续用率及避孕效果均高于元宫IUD及TCu220c IUD,更适合有剖宫史妇女放置。

Objective ：To investigate the clinical effects of the GeneFiex, YuangongIUD and Tcu220c placement after ce- sarean section. Methotls：380 cases with cesarean section history were included in this study in 2008, and placed ran- domly the GeneFiex, YuangongIUD and Tcu220c. After two year follow up, the effects of these IUDs were evaluated. Results：The pregnancy rate with GeneFiex in situ was 0.85%, expulsion rate was 0.85%, discontinuation rate was 1. 69%, and the continuation rate was 96.67% ; the pregnancy rate with YuangonglUD in situ was 5.79%, expulsion rate was 6.61%, discontinuation rate for symptom was 5.65%, and the continuation rate was 83.03% ; and the pregnancy rate with Tcu220c insitu, expulsion rate, discontinuation rate, and the continuation rate for symptom were 5.83%, 6. 56%, 6.67%, and 82.31%, respectively. Conclusion ：The GeneFiex is the low occurrence rats of bleeding, abdominal pain and expulsion, the high continuation rate, and the better contraceptive effect, compared with YuangonglUD and TCu220c, which could be more suited to the women with history of previous cesarean section.
