乳腺癌患者P-gp表达与临床病理特征及化疗后血液毒性的关系 被引量:2

The Relationship between the Expression of Multidrug Resistance P-Glycoprotein and Blood Toxicity after Chemotherapy in Patients with Breast Cancer

摘要 目的检测多药耐药基因产物P-糖蛋白(P-glycoprotein,P-gp)在乳腺癌组织中的表达,分析其与乳腺癌的临床病理特征及化疗后出现的血液毒性之间的关系。方法收集宁夏医科大学总医院154例乳腺癌患者的术后石蜡包块及相关临床及病理资料;应用免疫组化方法检测乳腺癌组织中P-gp的表达,据此将患者分为P-gp阳性组和P-gp阴性组,分析其与临床病理指标及化疗后血液毒性的关系。结果①乳腺癌患者不同年龄、淋巴结转移状况、肿块大小、肿瘤组织学分型及不同的雌激素受体(Estrogen receptor,ER)、孕激素受体(Rrogesterone receptor,PR)和人表皮生长因子受体2(C-erbB-2 receptor、C-erbB-2)表达状态,其P-gp表达差异均无统计学意义(P>0.05);②P-gp阳性组化疗后白细胞和血红蛋白的Ⅰ~Ⅳ度下降率分别为79.44%和43.93%,均低于P-gp阴性组化疗后白细胞和血红蛋白的Ⅰ~Ⅳ度下降率95.74%及63.83%,其差异均有统计学意义(P<0.05);③P-gp表达阳性组和阴性组化疗后的血小板下降率及血三系Ⅳ度下降率之间的差异均无统计学意义(P均>0.05)。结论多药耐药基因产物P-gp尚不能作为预测乳腺癌预后的不良因素及选择激素治疗的参考指标,但可作为预测化疗后出现血液毒性的指标。 Objective To explore diagnositic value of narrow-band imaging with magnifying endoscopy on morphological changes of gastric pits in gastric lesions and early gastric cancer.Methods 112 cases with local mucosal lesions including erosion,abnormal color,uplift,shallow depression and rough were recruited in this study.Patterns of gastric pits were observed with NBI-ME model and,the lesion type was determined with reference to Tanaka.Results Using narrow band imaging and magnifying endoscopy,the detecting rate gastric precancerous lesions including severe intestinal metaplasia and mild-moderate dysplasia was 39.2%(44/112) and the detecting rate of early gastric cancer was,42.3%(11/26).Conclusion Narrow band imaging and magnifying endoscopy can improve the detecting rate of precancerous lesions and early gastric cancer.
