血清嗜酸性粒细胞阳离子蛋白在慢性鼻窦炎伴变应性鼻炎患者治疗中的变化 被引量:2

Changes in Serum Eosinophile Cationic Protein in Treatment of Chronic Rhinosinusitis Accompanied by Allergic Rhinitis

摘要 目的分析慢性鼻窦炎伴有变应性鼻炎患者在行功能性鼻窦内镜手术(FESS)基础上接受特异性免疫治疗(ASIT)前后血清嗜酸性粒细胞阳离子蛋白(ECP)的变化,评价免疫治疗的效果。方法将54例慢性鼻窦炎伴变应性鼻炎患者按过敏原的不同分为尘螨组(n=29)及非尘螨组(n=25)。尘螨组采用FESS+术后ASIT治疗;非尘螨组采用FESS+术后布地奈德喷鼻治疗。分别对2组患者在治疗前后ECP值的变化进行比较。结果治疗前尘螨组和非尘螨组血清ECP分别为(25.7±3.1)μg·L-1和(24.8±2.7)μg·L-1,2组比较差异无统计学意义(P>0.05);治疗后尘螨组和非尘螨组血清ECP分别为(18.6±2.8)μg·L-1和(21.8±1.4)μg·L-1,尘螨组明显低于治疗前及非尘螨组(均P<0.05)。结论 ASIT能有效地抑制嗜酸性粒细胞在鼻腔的聚集与活化,降低其释放ECP的能力,有效地降低患者血清ECP。通过对血液中ECP的检测可对疾病预后进行判断,对ASIT的疗效进行评价。 Objective To evaluate the effects of functional endoscopic sinus surgery(FESS) combined with allergen-specific immunologic therapy(ASIT) on chronic rhinosinusitis accompanied by allergic rhinitis by observing the changes in serum eosinophile cationic protein(ECP).Methods Fifty-four patients with chronic rhinosinusitis accompanied by allergic rhinitis were divided into two groups according to dust mite allergens.Dust mite group(n=29) was treated with FESS combined with postoperative ASIT and non-dust mite group(n=25) was treated with FESS associated with postoperative budesonide nasal spray.The changes in serum ECP after treatment were compared between the two groups.Results No siginifant differences in the levels of serumal ECP were observed before treatment between dust mite group and non-dust mite group [(25.7±3.1)μg·L-1vs(24.8±2.7)μg·L-1,P0.05].After treatment,serumal ECP levels in dust mite group were lower than those in non-dust mite group[(18.6±2.8)μg·L-1vs(21.8±1.4)μg·L-1,P0.05].Conclusion ASIT can decrease the levels of serumal ECP through reducing eosnophile granulocyte aggregation and activation.The detection of blood ECP may evaluate the prognosis of chronic rhinosinusitis accompanied by allergic rhinitis and the efficacy of specific immunotherapy.
