宫颈液基细胞学检查在宫颈病变诊断中的应用价值

Value of liquid-based cervical cytology in the diagnosing for cervical intraeithelial neoplasia

目的探讨宫颈液基细胞学对CIN诊断的临床价值。方法回顾性分析2008年12月～2010年12月346例患者行阴道镜宫颈活组织检查结果异常的临床资料。检查结果以病理结果为金标准进行分析。结果 173例ASC-US中诊断为CIN 86例,与宫颈活检符合率为49.7%(CINⅠ71例,CINⅡ11例,CINⅢ4例);诊断为浸润癌1例,与宫颈活检符合率为0.6%。25例ASC-H中诊断为CIN 18例,与宫颈活检符合率为72.0%。105例LSL中诊断为CIN 72例,与宫颈活检符合率为68.6%。43例HSL中诊断为CIN 33例,与宫颈活检符合率为76.7%;诊断为浸润癌5例,与宫颈活检符合率为11.6%。结论宫颈液基细胞学是宫颈癌前病变简单有效的诊断方法,值得临床推广应用。

Objective To study on value of liquid-based cervical cytology in the diagnosing for cervical intraeithelial all neoplasia （CIN）. Methods The clinical data of 346 cases from December 2008 to December 2010 was analyzed retrospectively, and all the patients were given the test liquid-based cervical cytology combined with cervical biopsy histopathology. The results of liquid-based cervical cytology was judged through the pathology results as the gold standard. Results There were 86 cases CIN patients among 173 cases with ASC-US patients, the compliance rate with the pathology was 49.7% （71 cases CIN I , 11 cases CIN II and 4 cases CINm） and 1 cases was diagnosed invasive carcinoma. There were 18 cases CIN patients among 25 cases with ASC-H patients, the compliance rate with the pathology was 72.0%. There were 72 cases CIN patients among 105 cases with LSL patients, the compliance rate with the pathology was 68.6%. There were 33 cases CIN patients among 43 cases with HSL patients, the compliance rate with the pathology was 76.7%. 5 cases was diagnosed invasive carcinoma, the compliance rate with the pathology was 11.6%. Conclusion The liquid-based cervical cytology is simple and effective diagnostic method for the cervical lesions and is worthy of clinical application.
