股骨粗隆间骨折内固定与人工髋关节置换术的效果比较及围术期护理

The effect and perioperative care of intertrochanteric fractures internal fixation and the hip arthroplasty comparative

目的:探讨股骨粗隆间骨折内固定与人工髋关节置换术的效果比较及围术期护理。方法:将本院2009年5月～2011年4月收治的80例股骨粗隆间骨折患者随机分为观察组与对照组,每组各40例,观察组给予股骨粗隆间骨折内固定治疗,对照组给予人工髋关节置换术治疗,观察组给予术前、术后全面护理,观察两组的疗效。结果:两组患者的手术时间、术中输血量比较差异无统计学意义。对照组患者的术后卧床时间为(4.52±3.25)周,并发症发生率为5.0%,观察组患者的术后卧床时间为(12.01±1.51)周,并发症发生率为25.0%,两组间比较差异有统计学意义。结论:人工髋关节置换术的术后卧床时间短于股骨粗隆间骨折内固定,并发症发生率低,术后功能恢复亦比股骨粗隆间骨折内固定更好,积极的围术期护理在保证手术效果方面有重要作用。

Objective： To study the different effects and perioperative care of intertrochanteric fractures internal fixation and the hip arthroplasty comparative. Methods： Eighty cases intertrochanteric fractures patients from May 2009 to April 2011 in our hospital were randomly divided into observation group and control group, each group of 40 cases, the observation group was given intertrochanteric fractures internal fixation, and the control group was given hip arthroplasty treatment, observation group were given and postoperative nursing comprehensive. The curative effects of two groups were observed. Results： The differences of operation time and operation in blood transfusions of two sets of patients were statistically significant. The time in bed after surgical operation of patients in the control group were （4.52±3.25） weeks and the complication rate was 5.0%, the time in bed after surgical operation of patients in the observation group were （12.01±1.51） weeks and the complication rate was 25.0%, the difference between the two groups was significant. Conclusion： The time in bed after surgical of intertrochanteric fractures internal fixation is shorter than the hip arthroplasty comparative, and the incidence rate of complicating disease is lower, the function recovery is also better than intertrochanteric fractures internal fixation. Active perioperative care has an important role in ensuring the effect of operations.
