天津市海河医院结核合并2型糖尿病病例资料流行病学分析

Epidemiological Analysis of Tuberculosis Patients Complicated with Type 2 Diabetes Mellitus in Tianjin Haihe Hospital

目的了解结核合并2型糖尿病(TB-T2DM)流行病学情况,为制定相关的防治策略提供科学依据。方法统计分析天津市海河医院预防科2007-2010年TB登记资料。结果 2007-2010年TB-T2DM患者227例,占同期登记TB患者比例4年平均为8.69%,平均年登记率为0.47/10万,4年间无明显的上升或下降趋势。男性TB-T2DM患者占TB登记人数中男性患者的构成比为8.28%,女性TB-T2DM患者构成比为9.51%,差异无统计学意义(χ2=1.102,P>0.05)。TB-T2DM患者年龄22～89岁,以中老年患者为主,40岁后两病并发率大幅上升。在TB-T2DM患者中,痰涂阳性者137例(60.35%)、涂阴77例(33.92%),出现空洞43例(18.94%)。TB-T2DM患者的涂阳率(χ2=72.648,P<0.01)和空洞率(χ2=35.266,P<0.01)均高于无合并T2DM的TB患者。结论 TB-T2DM发生率较高,以中老年发病为主,涂阳率和空洞率均较高,给结核病控制带来一定的困难,所以要加强这一人群的防治工作。

Objective To understand epidemiologieal analysis of tuberculosis patients complicated with type 2 diabetes mellitus（TB-T2DM） in Tianjin Haihe Hospital. Methods The registration materials of TB-T2DM patients recorded from 2007 to 2010 in prevention division of Tianjin Haihe Hospital were analyzed. Results The total number of TB-T2DM patients were 227 from 2007 to 2010, accounted for 8.69% of total TB patients .The average annual registration rate was 0.47/100 000 and there was no obvious rise or decline trend during these 4 years. TB-T2DM proportion of the male accounted for 8.28% and the female for 9.51%, and there was no significant difference between gender （X2= 1.102,/：＇〉0.05 ）. The age of TB-T2DM patients ranged from 22 to 89 years, majority was in middle-aged and elderly patients aged over 40 years old. Among TB-T2DM patients, 137 （60.35%） cases were smear positive, 77 cases were smear negative （33.92%）, while 43 cases were with cavity （ 18.94% ）. All these proportions were significantly higher than those in riB patients without diabetes （X2=72.648, P〈0.01 and X2=35.266, P〈0.01 ）. Conclusion The TB-T2DM incidence is relatively high, and it mainly occurred in middle-aged and elderly cases. The proportions of both smear positive and with cavity were high. Therefore it is needed to strengthen TB control work in this population.
