微血管减压术治疗三叉神经痛的临床研究

The clinical research of microvascular decompression for trigeminal neuralgia

目的探讨微血管减压术治疗三叉神经痛的手术效果、并发症和副作用,提出改良方案,提高对该病的诊疗效果。方法回顾性分析本院2008年8月～2011年4月收治住院的采用微血管减压手术治疗的原发性三叉神经痛患者74例,对手术技术及血管的处理方法进行改良,观察其治疗效果。结果手术的总有效率为98.6%,无一例发生脑脊液漏、听力丧失、颅内感染等严重并发症,术后随访发现复发率1.4%。结论微血管减压术从根本上解除了三叉神经痛的常见病因,术后完全保留了三叉神经各分支的功能,是目前手术治疗三叉神经痛的有效方法之一。

Objective To investigate the surgery effect, complications and side effect of microvascular decompression for trigeminal neuralgia, propose improvement programs and improve the clinical effect. Methods Seventy four patients with primary trigeminal neuralgia in our hospital from August 2008 to April 2011 which received the microvascular decompression surgery were retrospectively analysed. Results The total effective rate of this surgery was 98.6%, with no cerebrospinal fluid leakage, hearing loss, intracranial infections and other serious complications, it found that the recurrence rate was 1.4% according to postoperative follow-up. Conclusion Microvascular decompression is an effective method of surgical treatment of trigeminal neuralgia, it can relieve common causes of trigeminal neuralgia, preserve function of all the trigeminal nerve branches, is one of the effective operation method to trigeminal neuralgia.
