参麦注射液对先天性心脏病体外循环术后血浆神经肽Y的影响

摘要 目的:评价参麦注射液对体外循环术后血浆神经肽Y(Neuropeptide Y,NPY)的影响,探讨其对体外循环手术的心肌保护作用机理。方法:60例简单先天性心脏病行体外循环手术患者,均为室间隔缺损、房间隔缺损,非重度肺动脉高压;年龄2岁~30岁,随机分为2组,对照组(n=30)按照常规行体外循环手术,治疗组在对照组基础上予参麦注射液1ml(/kg.d)+10%葡萄糖注射液1ml(/kg.d),ivdripqd×3天(术前2天+术后1天)。分别测定术前(T0)、体外循环结束时(T1)、结束后3小时(T2)、结束后24小时(T3)的NPY浓度。结果:术后两组血浆神经肽Y浓度均较术前增高,随后回落;但实验组升高的幅度明显低于对照组(P<0.05)。结论:参麦注射液可通过降低体外循环手术炎症反应起到一定的心肌保护效果。 Objective: To evaluate the influence of Shenmai injection on plasma neuropeptide Y in patients with congenital heart disease after cardiopulmonary bypass,and to explore its action mechanism of myocardial preservation in eardiopulmonary bypass surgery. Methods: 60 cases of congenital heart disease with cardiopulmonary bypass were randomly divided into control group and experimental group including ventrieular septal defect patients, atrial septal defect and non-severe pulmonary hypertension patients age from 2 to 30 years old. Control group (n=30) was routine cardiopulmonary bypass surgery,and experimental group on the basis of control group was extrally added Shenmai injection lml/(kg'd)+10%GS lml/(kg'd),iv drip,qdx3d (2 days before operation+l yday after operation)in which plasma neuropeptide Y were respectively detected in T0(before operation),T1 (when end extracorporeal eireulation),T2(3 hours after operation) and T3 (24 hours after operation). Results: Plasma neuropeptide Y was increase after operation than that before operation in two groups,but increase range of experimental group was obviously lower than that of control group(P〈0.05). Conclusion: Shenmai injection can reduce inflammatory response during eardiopulmonary bypass surgery,which may be a certain degree of myocardial protection.
