EMRL治疗上消化道粘膜下肿瘤52例体会

Analysis of EMRL on treatment for 52 patients with upper gastrointestinal submucosal tumors

目的:回顾性评价内镜下粘膜套扎切除术(EMRL)在治疗上消化道粘膜下肿瘤(SMTs)的价值和安全性。方法:术前用超声内镜确定病灶范围及浸润范围的上消化道粘膜下肿瘤患者用EMRL。结果:EMRL对<20mm的SMT完全切除率达100%,>20mm的SMTs胃镜病理与外科手术病理诊断符合率达90%;除4例发生消化道出血经胃镜下治疗后停止外,无其它严重并发症发生。结论:EMRL对小的SMTs有较高的完全切除率,>20mm的大SMTs能获得较理想的组织学依据;而且简单、方便、经济、有较低的并发症发生。

Objective： To evaluate retrospectively the efficacy and safety of endoscopic mucosal resection with ligation device （EMRL） in treatment for patients with upper gastrointestinal submucosal tumors （SMTs）. Methods： Gastroscopy and endoscopic uhrasonography were used to evaluate the origin, extent, infiltrating depth and properties of 52 patients with SMTs which were treated by EMRL Results： Complete resection with the size lesser 20mm of SMTs were achieved in which total resection rate was 100% ,diagnose accordance rate between endoscopic pathology and surgical pathology was 90%.There was no other serious complications except 4 patients with bleeding during the procedure. Conclusion： EMRL techniques have high total resection rate, and can acquire ideal histological basis in 〉 20mm SMTs,which are simple, convenient,economic and less complications.
