结直肠癌合并肠梗阻老年患者98例临床分析

Clinical analysis of senile patients with colorectal cancer combined with intestinal obstruction in 98 cases

目的探讨结直肠癌合并肠梗阻老年患者手术治疗的方法及效果。方法将本院近年来收治的结直肠癌合并肠梗阻98例老年患者的临床资料进行回顾性分析,了解其手术治疗效果及术后并发症的发生情况。结果 98例患者中,95例患者痊愈出院,住院时间10～31d,3例患者死亡,死亡原因中,并发心肌梗死1例,感染中毒性休克2例。98例患者中共发生术后并发症12例,其中一期吻合患者并发症发生率为10.0%;近端肠管于腹壁切口造瘘者并发症发生率为33.3%。结论对于结直肠癌合并肠梗阻老年患者,尽量行肠管一期吻合术,既可避免二次创伤,又可减轻患者心理障碍。

Objective To investigate the operative methods and efficacy of the senile patients with colorectal cancer combined with intestinal obstruction.Methods The clinical data of 98 cases of senile patients with colorectal cancer combined with intestinal obstruction were retrospectively analyzed,then the efficacy of the operation and postoperative complications were known.Results In the 98 cases of patients,95 cases recovered and discharged,their hospitalization time were 10-31 days,3 cases died,cause of death of 1 case was postoperative myocardial infarction,2 cases were toxic shock.Complications occurred in 12 cases of 98 cases,complication rate of the patients treated by one-stage anastomosis was 10.01%;complication rate of the patients treated by proximal intestine building fistula at abdominal wall was 33.3%.Conclusion One-stage intestine anastomosis should be treated to the senile patients with colorectal cancer combined with intestinal obstruction as possible,it can not only avoid secondary insult,but also can reduce the psychological obstacle.
