肾细胞癌内皮抑素的表达与微血管密度相关性研究

摘要 目的探讨内皮抑素表达与肾细胞癌微血管密度及预后的关系。方法采用免疫组化的方法检测45例原发性肾细胞癌组织及35例癌旁正常肾组织中的Endostatin及CD34,并观察Endostatin的表达同MVD的关系。结果肾细胞癌组织中Endostatin阳性表达率为77.8%,癌旁正常肾组织为31.4%,差异有统计学意义(P<0.01),在Ⅲ~Ⅳ期病例中的表达显著强于Ⅰ~Ⅱ期(P<0.01)。肾细胞癌的MVD(32.89±9.42)明显高于癌旁正常肾脏组织(16.20±8.31)(P<0.01);Ⅲ~Ⅳ期MVD(43.86±9.11)显著高于Ⅰ~Ⅱ期(28.71±8.04)(P<0.01),且与Endostatin表达具有相关性(P<0.05);MVD在组织学分级之间差异有统计学意义(P<0.05)。结论 Endostatin在肾癌中表达显著高于癌旁的正常肾组织,且与MVD显著相关。MVD与肾癌的临床分期、病理分级密切相关。 Objective To determine the relation of Endostatin expression with MVD and prognosis in renal cell carcinoma(RCC). Methods With anti endostatin McAb( mulficlone antibody) and anti CD34 McAb,lmmunohistochemistry was used to study the expres sion of endostatin and the tumor MVD in 45 cases of RCC and in 35 caSes of normal renal tissue as controls. Results The expression ratios of endo statin in RCC tissue and in normal kidney were 77.8% and 31, 4% respectively, showing significant difference( P 〈 0. 01 ). The en dostatin expression in 45 cases of carcinoma was associated with clinical stages ( P 〈 0. 05 ), while no significant difference was detected in sex, age, the size of tumor and histologic grade. The mean MVD in RCC and in normal tissue was ( 32.89 ~ 9. 42 ) and ( 16.20 ~ 8.31 ) re spectively, showing significant difference ( P 〈 0.01 ). In 45 cases of carcinoma, the mean MVD in stage Ill ~ IV (43.86 ~ 9. 11 ) was much higher than in stage I ~ 11 ( 28.71 ~ 8.04) ( P 〈 0. 01 ). but had no significant difference in sex, age, size of tumor. There were statistically significant correlation between Endostatin and MVD. Conclusion The positive expression of Endostatin is significantly related with MVD and tumor progression and poorer prognosis in RCC. Quantification of Endostatin may provide valuable markers for a poorer prognosis of pa tients.
