摘要
目的:探讨综合护理干预对无创正压通气(NIPPV)治疗慢性阻塞性肺疾病急性加重期(AECOPD)并发Ⅱ型呼吸衰竭患者的影响。方法:将90例AECOPD并发Ⅱ型呼吸衰竭患者随机分为干预组和对照组各45例,对照组给予NIPPV治疗和常规护理,干预组在此基础上给予综合护理干预。观察比较两组患者治疗依从性、人机协调时间和动脉血气变化情况。结果:干预组治疗依从性、人机协调时间均明显优于对照组(P<0.05);干预组PaCO2较对照组明显下降(P<0.05),pH值、PaO2、SaO2较对照组明显升高(P<0.05)。结论:综合护理干预能显著提高患者的治疗依从性,缩短达到人机协调的时间,提高疗效,是NIPPV治疗成功的关键。
To investigate the effect of comprehensive nursing intervention on the patients with complicated type II respirato- ry failure in the acute exacerbation stage of the treatment of COPD with non - invasive positive pressure ventilation NIPPV. Methods ： 90 AECOPD patients with type ]] respiratory failure were randomly divided into intervention and control groups （45 patients in each group）. The patients in the control group received NIPPV treatment and routine nursing care, and comprehensive nursing intervention was provided for the patients in the intervention group on this basis of the routine nursing care. The treatment compliance, the time to achieve patient - ventilator interaction and the change of arterial blood gas were observed and compared between the two groups. Results： The treatment compliance, the time to achieve patient - ventilator interaction were significantly better in the intervention group than the control group （ P 〈 0.05 ） ; PaCO2 significantly decreased in the intervention group compared with the control group （ P 〈 O. 05 ） ; pH values, PaO2, SaO2 were significantly higher in the intervention group than the control group （ P 〈 O. 05 }. Conclusion ： The comprehensive nursing intervention can significantly improve the patient~ compliance, shorten the time to achieve patient -ventilator interaction so as to improve the efficacy of NIPPV treatment. It is the key to success of the treatment.
