多巴胺、多巴酚丁胺联合治疗顽固性心力衰竭疗效分析

Efficacy analysis of combination therapy of dopamine and dobutamine for intractable heart failure

目的观察多巴胺、多巴酚丁胺联合治疗顽固性心力衰竭的临床效果。方法将2008年1月～2011年2月笔者所在医院收治的50例顽固性心力衰竭患者随机分为治疗组和对照组各25例,对照组给予利尿、吸氧、营养心肌、ACEI类药及洋地黄等常规治疗,治疗组在常规治疗基础上给予小剂量多巴胺、多巴酚丁胺联合治疗,对比观察两组临床效果。结果治疗组左室射血分数、左室心输出量均高于对照组,且组间比较差异具有统计学意义(P<0.05);治疗后,治疗组有效率为92.0%,对照组有效率为64.0%,两组有效率比较差异具有统计学意义(P<0.05)。结论多巴胺、多巴酚丁胺联合治疗顽固性心衰,能够有效改善心功能、控制心衰,临床有效率高,值得临床推广应用。

Objective To observe the clinical effect of dopamine combining with dobutamine for intractable heart failure. Methods 50 cases with intractable heart failure treated in our hospital from January 2008 to February 2011 were randomly divided into treatment group （n=25） and control group（n=25）. Control group was given conventional therapy of diuretic, oxygen, nutrition, cardiac, ACEI drugs, digitalis, etc. Treatment group was given combination therapy,of low-dose dopamine and dobutamine on the basis of conventional therapy. To compare the clinical effects of the two groups. Results The leftventricular ejection fraction and left ventricular cardiac output of the treatment group were higher than those in control group. Difference was statistically significant between the two groups（P 〈 0.05）.After treatment, the effective rate of treatment group was 92.0%, and the effective rate was 64.0% in control group. Efficiency difference between the two groups was statistically significant （P 〈 0.05）. Conclusion Dopamine combining with dobutamine can improve cardiac function and control heart failure for intractable heart failure, which has a high clinically effective rate and is worthy of clinical application.
