慢性阻塞性肺病患者血清脑钠肽水平变化及临床意义研究

Prognostic value and clinical significance of plasma brain natriuretic peptide levels in patients with COPD

目的探讨慢性阻塞性肺病患者血清脑钠肽水平的预测价值及临床意义。方法测定86例慢性阻塞性肺病血清脑钠肽(BNP)0～24 h,24～48 h和恢复期的水平,并与40例健康体检者(对照组)进行比较,分析慢性阻塞性肺病患者急性加重和恢复期的BNP水平。结果与对照组相比较,在0～24 h,24～48 h和恢复期时间段慢性阻塞性肺病患者血清BNP水平均显著升高(P<0.001);0～24 h,24～48 h血清BNP水平明显高于缓解期(P<0.001)。结论动态监测BNP的变化对慢性阻塞性肺病患者有较好的预测作用。

Objective To explore the prognostic value and clinical significance of brain natriuretic peptide（BNP） level in patients with chronic obstructive pulmonary diseases（COPD）.Methods Eighty-six patients（45 mechanical ventilation group and 41 noninvasive mechanical ventilation group）with COPD from June 2009 to March 2011 were studied and 40 volunteer with healthy examined people as controls participated in this study.Plasma BNP concentrations were measured on 0~24 h,24~48 h and paracmasia.Results There was a statistical difference in plasma BNP concentrations both in group mechanical ventilation and noninvasive mechanical ventilation among 0~24 h,24~48 h and paracmasia（P0.001）,0~24 h and 24~48 h plasma BNP levels differed significantly in patients who presented with acute exacerbations compared with control group（P0.001）.Conclusion Serial determination of plasma BNP concentration has a better prognostic value for patients with COPD.
