慢性肥厚性鼻炎的四种改进术式疗效比较

Comparison of the curative effects of four kinds of improved operation type in treatment of chronic hypertrophic rhintis

目的比较四种改进术式与传统方法在慢性肥厚性鼻炎治疗中的优劣。方法将2010年6月~2011年1月本科诊断为慢性肥厚性鼻炎的280例患者分为四组。A组：82例行微波治疗;B组：83例行低温等离子消融治疗;C组：65例行下鼻甲骨-黏膜剥离术治疗;D组：50例行下鼻甲骨骨折术治疗。结果四组中A组短期疗效好,B组创伤小、恢复快、长期效果好,C组长期效果好,但创伤大、操作稍复杂,D组长期效果好、创伤小、操作简单。A、B、C、D组6个月后总有效率分别为60.98%、84.34%、86.15%、82.00%。A组长期总有效率与其他比较差异均有高度统计学意义（均P〈0.01）。其他三组（B、C、D）间比较差异无统计学意义（P〉0.05）。四组均无明显后遗症。结论低温等离子消融及下鼻甲骨骨折治疗慢性肥厚性鼻炎有良好临床疗效,可于临床广泛应用。

Objective To compare four kinds of improved operation type in treatment of chronic hypertrophic rhinitis with traditional methods of quality.Methods 280 cases with chronic hypertrophic rhinitis from June 2010 to January 2011 were divided into four groups randomly.Group A： 82 cases of chronic hypertrophic rhinitis（HR） were treated by microwave.Group B：83 cases of HR were treated by frequency ablation temperature-controlled radio.Group C： 65 cases of HR were treated by inferior nasal concha mucosal dissection.Group D： 50 cases of HR were treated by inferior turbinate bore fracture.Results Among four groups,the short term effect was good in group A;group B was less trauma,quicker recovery,the long term effect was good.The trauma was large,the long term effcet was good and more complex operation in group C;group D had the advantage of long-term good effect,less trauma,simple operation.Four groups of HR were followed up half year after operation,effective rate was 60.98%（group A）,84.34%（group B）,86.15%（group C）,82.00%（group D） respectively.Group A compared with group B,group C and group D,effective rate had significant difference among them（all P 0.01）.The other 3 groups（B,C,D） effective rate had no significant difference among them（P 0.05）.There was no significant sequela in four groups.Conclusion Frequency ablation temperature-controlled radio and inferior turbinate bore fracture in treatment of chronic hypertrophic rhinitis have obvious clinical,and can be used to the clinical application.
