我院住院西药房退药现状及原因分析

Analysis of the current situation of drug-return and causes in the pharmacy department of our hospital

目的了解我院住院西药房退药原因并采取有效防范措施,促进医院合理用药,提高药房管理水平。方法收集我院2011年8月住院部西楼药房退药处方580张,以《医疗机构药事管理规定》为依据,从退药科室、退药原因、退药品种数角度进行统计分析并提出建议。结果退药处方最多的科室为小儿内科,共124张,占21.4%,其次为神经内三科,共107张,占18.4%;退药金额最多的是放疗科,共26 769.564元,占28.9%;退药原因中的关键因素是医师原因。结论医院应加强管理,制订严格的退药制度,引入奖惩制度,从医师、药师、患者三方面入手,加强其沟通,严格退药手续,防止因退药造成的医疗差错,同时应强化医生合理用药意识,提高合理用药水平,完善医德教育,从而有效控制退药。

Objective To understand the reason of drug-return in pharmacy department of our hospital and take the effective preventive measures,in order to promote the hospital rational drug use,improve the pharmacy management level.Methods 580 copies of drug-return prescription were collected in pharmacy department of our hospital in August 2011.Basis on the pharmacy management regulations of medical institutions,the drug-return departments,drug-return reasons and drug-return species were analyzed statistically in this paper and put forward suggestions.Results Pediatric medicine of 124 copies was the most drug-return prescription department,accounted for 21.4%;neurology medicine department of 107 copies was followed,accounted for 18.4%;the largest amount of drug-return department was radiotherapy department,with 26 769.564 yuan,accounted for 28.9%;the key factor of the drug-return reasons was the physicians reason.Conclusion The hospital management should be strengthened,formulate strict system of drug-return,introduce rewards and punishment system,strengthen the communication among physicians,pharmacists and patients,strictly regulate the drug-return formalities,in order to avoid malpractice caused by drug-return,meanwhile,also should strengthen the consciousness of rational drug use,improve rational drug use level,perfect medical ethics education,thus effectively control drug-return.
