摘要
目的:通过测定子宫内膜异位症(EMs)患者血清中骨桥蛋白(OPN)的含量,探讨OPN在EMs发病机制中的作用及血清OPN对EMs诊断、鉴别诊断的价值。方法:采用ELISA法检测30例卵巢EMs患者及30例非EMs的卵巢良性肿瘤患者术前、术后血清中OPN的含量。结果:①研究组术前血清OPN水平为(20.80±8.79)ng/ml,高于对照组及术后(P<0.01);对照组术前、术后血清OPN水平差异无统计学意义(P>0.05)。②R-AFS期别增高,血清OPN水平也增高,Ⅲ期较Ⅱ期、Ⅳ期较Ⅲ期EMs患者血清OPN水平明显升高〔(18.89±3.75)ng/ml vs(11.55±2.23)ng/ml,P<0.05,(28.78±8.44)ng/ml vs(18.89±3.75)ng/ml,P<0.01〕。血清OPN水平与R-AFS分期间有相关性(r=0.771,P<0.01)。结论:血清OPN水平在EMs辅助诊断中具有一定的应用价值,监测OPN的动态变化可能有利于判定EMs的手术疗效及预后。血清OPN水平与R-AFS评分之间有一定的相关关系,可能与疾病的严重程度有关,亦可能参与了EMs的发生、发展过程。
Objective:To explore the effect of osteopontin in the pathogenesis of endometriosis,the value of serum osteopontin for diagnosis and differential diagnosis of endometriosis by detecting the serum contents of osteopontin in the patients with endometriosis.Methods:The serum contents of osteopontin in 30 patients with ovarian endometriosis and 30 patients with ovarian benign tumors(not endometriosis) before and after operation were detected by enzyme-linked immunosorbent assay(ELISA).Results:The serum osteopontin level in study group before operation was(20.80±8.79) ng/ml,which was significantly higher than those in control group and in study group before operation(P0.01).In control group,there was no significant difference in serum osteopontin level before and after operation(P0.05).The expression level of osteopontin increased with the R-AFS stages;among the patients with endometriosis,the serum osteopontin level in stage Ⅲ was(18.89±3.75) ng/ml,which was significantly higher than that in stage Ⅱ((11.55±2.23) ng/ml)(P0.05);the serum osteopontin level in stage IV was(28.78±8.44) ng/ml,which was significantly higher than that in stage Ⅲ((18.89 ± 3.75) ng/ml)(P0.01).There was a correlation between serum osteopontin level and R-AFS stages(γ=0.771,P0.01).Conclusion:Serum osteopontin detection has a certain application value for assisted diagnosis of endometriosis,monitoring the dynamic changes of osteopontin may be helpful to predict the clinical efficacy of endometriosis surgery and prognosis.There is a certain correlation between serum osteopontin level and R-AFS stages,which may be related to the severity of the disease,and it may participate in the occurrence and development of endometriosis.
