微量泵持续泵奶喂养联合腹部按摩应用于早产儿的效果

目的探讨通过微量泵口饲管持续均匀泵奶喂养联合腹部按摩在早产儿护理中的应用效果。方法将60例早产儿随机分为实验组和对照组各30例，两组均在常规治疗及护理的基础上采用微量泵通过口饲管持续均匀泵奶喂养法，实验组在喂养过程中每2～3小时给予腹部按摩1次，观察5d中两组早产儿排便、胃残余奶量、呕吐、腹胀、每日体重增长、黄疸消退情况、出现低血糖的情况。结果实验组早产儿首次排便时间1d以上5例，对照组12例，两组比较差异有统计学意义（x^2=4．02，P〈0．05）；实验组早产儿第2天黄疸指数（123．44±10．25）μmol／L，对照组（128．76±11．31）μmol／L，两组比较差异有统计学意义（t=1．91，P〈0．05）；实验组早产儿出现低血糖2例，对照组8例，两组比较差异有统计学意义（x^2=4．32，P〈0．05）。结论微量泵持续泵奶喂养联合腹部按摩有利于早产儿胃肠功能的成熟和营养吸收，增加体重，提高其喂养耐受性。

Objective To explore the effect of the pump through the mouth feeding tube continuous uniform pump milk combined with abdominal massage used in preterm infants nursing. Methods 60 cases of premature infants were randomly divided into experimental grqup and control group, two groups were in the routine treatment and nursing care on the basis of the pump through the mouth feeding tube continuous uniform pump milk feeding method, the experimental group was given 2 - 3 H feeding process of each abdominal massage, control group were not given the abdominal massage. Defecation, gastric residual milk, vomiting, bloating, daily weight gain, jaundice cases, number of cases of hypoglycemia were observed in 5 days in the two groups： Results The first time feces of 5 cases was longer than one day in experimental group, and 12 cases in the control group （ X2.=4.02, P 〈 0. 05 ） ; jaundice index in two days was （ 123.44 ±10.25 ） μmol/L and （ 128.76 ±11.31 ） μmol/L in the control group, and the difference was significant （ t = 1.91, P 〈 0. 05 ） ; 2 cases occurred hypoglycemia in experiment group, and 8 in the control group, and the difference was significant （ X2 = 4.32, P 〈 0. 05 ）. Conclusions Micro pump mouth continued feeding of milk combined with abdominal massage help keep gastrointestinal function in premature children and nutrient absorption, increase weight, improve feeding tolerance, and promote growth and development.
