摘要
目的探讨癌胚抗原(CEA)、铁蛋白、腺苷脱氢酶(ADA)以及抗结核抗体在胸腔积液性质诊断中的意义。方法对2010年1月—2011年9月胸腔积液患者107例(恶性积液39例、结核性积液43例、炎性积液25例),进行胸腔穿刺抽液,将胸腔积液分别进行CEA、铁蛋白、ADA、抗结核抗体进行测定。并根据设定的特殊临界值划分阳性阴性。结果 CEA敏感性特异性有效性分别为(51.3%,98.5%,81.3%),铁蛋白敏感性特异性有效性分别为(56.4%,89.7%,77.6%),ADA敏感性特异性有效性分别为(48.8%,98.4%,78.5%),抗结核抗体敏感性特异性有效性分别为(51.2%,89.1%,73.8%)CEA与铁蛋白联合检测敏感性特异性有效性分别为(92.3%,88.2%,90.7%),ADA与抗结核抗体联合检测敏感性特异性有效性分别为(90.7%,87.5%,88.8%)。结论 CEA和铁蛋白在癌性胸水诊断中具有至关重要的意义;ADA、抗结核抗体是结核性胸膜炎特征性标志物。采用联合检测的方法可以有效提高检出率,减少漏诊发生。
