实施系统管理促进肺结核患者治愈率提高的效果分析

Analysis on the impact of systematic management on increasing the cure rate of pulmonary tuberculosis patients

目的分析实施系统管理对肺结核患者治愈率的影响。方法对比分析河北省承德市新痰涂片阳性肺结核患者实施系统管理前(1994—2002年,6745例)和实施系统管理后(2003—2010年,11 426例)治疗管理的相关数据与治愈情况。结果承德市实施系统管理后,全程督导率由年平均71.55%(4826/6745)提高到92.69%(10 591/11 426);完成疗程率由年平均4.57%(308/6745)降低到0.15%(17/11 426);丢失率由年平均5.69%(384/6745)降低到0.46%(52/11 426);系统管理率由年平均78.30%(5281/6745)提高到93.22%(10 651/11 426);规则治疗率由年平均81.94%(5527/6745)提高到93.18%(10 647/11 426);新痰涂片阳性患者治愈率由年平均81.84%(5520/6745)提高到93.06%(10 633/11 426)。结论实行系统管理综合措施,促进了承德市新痰涂片阳性肺结核患者治愈率的大幅度提高。

Objective To analyze the impact of systematic management on the cure rate of pulmonary tuber- culosis patients. Methods Data related the treatment management and treatment outcome of the new smear posi- tive TB patients were compared and analyzed before （from 1994 to 2002, 6745 cases） and after （from 2003 to 2010, 11 426 cases）the implementation of systematic management in Chengde city. Results After implementation of sys- tematic management, the annual average rate of full course supervision,systematic management rate, the standard- ized treatment rate, the cure rate of new smear positive TB patients increased from 71.55^（4826/6745）to 92.69 （10 591/11 426）, 78.30~//00 （5281/6745） to 93.22% （10 651/11 426）, 81.94% （5527/6745） to 93.18~ （10 647/ 11 426）,81.84o//oo（5520/6745） to 93.06^（10 633/11 426）, respectively~ the treatment completion rate and the loss rate decreased from 4. 57 ~ （308/6745） to 0.15 ~ （17/11 426）, 5.69 ~ （384/6745） to 0. 46 ~ （52/11 426）, respec- tively. Conclusion The implementation of the systematic management dramatically improves the cure rate of new smear positive TB patients in Chengde city.
