两种途径使用甲氨蝶呤治疗异位妊娠临床观察

Clinic effects of methotrexate on the treatment of ectopic pregnancies： comparison of two defferent routes of administration

目的探讨血管介入使用甲氨蝶呤（MTX）加明胶海绵栓塞（简称介入治疗）治疗异位妊娠及肌肉注射MTX（简称注射治疗）治疗异位妊娠的临床疗效。方法分析本院介入治疗异位妊娠46例和同期注射治疗异位妊娠121例的临床资料。结果介入组和注射组总有效率分别为93．5％和81．8％，介入治疗优于肌肉注射，差异有统计学意义。介入治疗宫颈妊娠1例，子宫瘢痕妊娠1例获得成功。结论子宫动脉介入灌注甲氨蝶呤加栓塞治疗输卵管异位妊娠是一种微创、安全、有效、人性化的治疗方法，此法可用于宫颈妊娠、子宫瘢痕妊娠。

Objective To compare the clinic effect of methotrexate on the treatment of ectopic pregnancy by selective arteria uterine cannula chemotherapy embolism and by muscle injection. Methods Clinical data of 46 cases of ectopic pregnancy treated with arteria uterine visualization and 121 cases treated with muscle injection were reviewed respec- tively. Results The rate of successful clinical cure of ectopic pregnancy with elective arteria uterine cannula chemotherapy was 93.5 %, compared with 81.8 % in the muscle injection group （P 〈 0.01 ）. Conclusions Selective arteria uterine cannula chemotherapy embolism in treating ectopic pregnancy has the advantages of small wounds, security and a higher cure rate.
