摘要
目的:探讨改良的小切口白内障囊外摘除并人工晶状体植入术的手术方法并评价其效果。方法:对300例(312眼)白内障行小切口囊外摘除,术中隧道外切口为位于上方角膜缘后2mm弦长为5.5mm的反眉弓巩膜切口,隧道长3mm,角膜缘3点及9点作侧切口。连续环形撕囊,水分离后使晶状体核转于前房,右手持晶状体圈匙进入核下方拖住核,左手持劈核器置于核的上方,将核劈为两半,分别取出。双手操作吸净皮质,植入人工晶体。结果:术后1周内裸眼视力高于0.5达86%,术后2个月裸眼视力高于0.5的为95%。术后1周、1个月、3个月的角膜散光与术前比较的差异分别为1.16±0.55D;1.05±0.48D;0.58±0.35D。结论:改良的小切口白内障囊外摘除并人工晶状体植入术效果良好,组织损伤轻、散光小、安全性高、费用低廉。
Objective：To discuss and analyze the surgical method of the modified small incision ECCE and IOL implantation and evaluate its curative effect.Methods：After ECCE with modified small incision,an organic Lens was implanted in 300 cases of cataracts（312eyes）.The superior inversed frown-shaped scleral tunnel incisions were 5.5mm long.The inner incisions were on the transparent cornea and the tunnels depth were about 3mm.Results The uncorrected vision of 86% eyes were better than 0.5 on the first postoperative week.Postoperative astigmatism absolute values of corneas were（1.16±0.55）D;（1.05±0.48）D;（0.58±0.35）D,different from preoperative in 1 week,1 month and 3 months.Conclusion：The modified small incision ECCE and IOL implantation was effective,light-injury,small-astigmatism,safe and economical.
