摘要
目的探讨动脉健康(AHP)技术评估颈动脉内-中膜的价值。方法利用AHP技术测量283例非动脉粥样硬化受检者(正常组235例和增厚组48例)的颈动脉内-中膜厚度(CIMT),获得颈总动脉内-中膜厚度平均-平均值(CCA-IMT-Mean)、颈总动脉内-中膜厚度平均-最大值(CCA-IMT-Max)、颈动脉内-中膜厚度平均-平均值(CIMT-Mean)、颈动脉内-中膜厚度平均-最大值(CIMT-Max)以及血管年龄(VA)等参数,并对参数进行分析研究。结果 AHP技术可用性高(≥90%)。CIMT随着年龄增长而增长,CCA-IMT-Mean和CIMT-Mean的上限为0.9 mm,CCA-IMT-Max和CIMT-Max的上限为1.2mm。CCA-IMT-Mean、CCA-IMT-Max、CIMT-Mean和CIMT-Max对诊断内-中膜增厚有很高的效能(r≥0.98,P=0.000),临界值分别为0.65、0.83、0.71、0.86。VA和CIMT相关性好(r>0.9,P=0.000),而正常组VA与生理年龄有相关性(r=0.608,P=0.000)。结论 AHP技术能提供评价颈动脉内-中膜的超声参数,为临床提供有效的监测指标。
Objective To investigate the advantages of assessing carotid intima-media layer with arterial health package(AHP).Methods The carotid intima-media thickness(CIMT) of 283 subjects(235 subjects in the normal group and 48 subjects in the thickened group) without atherosclerosis was measured with AHP.The parameters of the mean intima-media thickness of common carotid artery(CCA-IMT-Mean),the max intima-media thickness of common carotid artery(CCA-IMT-Max),the mean intima-media thickness of carotid artery(CIMT-Mean),the max intima-media thickness of carotid artery(CIMT-Max) and vascular age(VA) were obtained and analyzed.Results AHP had high applicability(≥90%).CIMT was increased with age.The upper limits of both CCA-IMT-Mean and CIMT-Mean were all 0.9mm,and those of CCA-IMT-Max and CIMT-Max were all 1.2mm.CCA-IMT-Mean,CCA-IMT-Max,CIMT-Mean and CIMT-Max had high efficiency in diagnosing thickened carotid intima-media layer(r≥0.98,P=0.000),and the cut-off value of them was 0.65,0.83,0.71 and 0.86 respectively.VA had significant correlation with CIMT(r〉0.9,P=0.000),while VA of normal group had correlation with age(r=0.608,P=0.000).Conclusion AHP can provide ultrasonic parameter for assessing carotid intima-media layer,moreover,it provides effective monitoring indicators for clinic.
