摘要
目的 调查老年男性人群血清活性维生素D含量和动脉硬化指标的相关性.方法 上海交通大学附属仁济医院老年科体检的60岁及以上男性137例,运用电化学发光法测定血清中活性维生素D含量,根据检测值将其分为高、中、低值3组.同时对所有患者进行问卷调查及详细的体格检查,包括年龄、身高、体重等一般情况,疾病史、钙磷代谢相关药物使用情况及肾功能、血脂、血糖等生化指标的检测,并测定动脉硬化指标,包括踝臂脉搏波传导速度 (baPWV) 和臂踝指数 (ABI).分析血清中活性维生素 D 浓度和上述指标的相关性.结果 3组人群的一般情况、相关药物使用情况、心血管危险因素、心血管系统疾病情况差异无统计学意义;baPWV值差异无统计学意义,而ABI在3组人群中差异有统计学意义(P=0.047),ABI和血清中活性维生素D的含量呈负相关(P=0.015).结论老年男性中,活性维生素D的含昔与baPWV无相关,而与ABI呈负相关关系,提示活性维生素D和早期动脉硬化有一定的相关性.
Objective To Study the relationship of serum 25-hydroxyvitamin D and arteriosclerosis index in elderly male persons. Methods 137 consecutive male persons older than 60 years for physical examination were enrolled in De- partment of Geriatrics. The concentration of serum 25-hydroxyvitamin D were measured by using electrochemical lumin- escence method. Then all the candidates were equally classified into three groups according to the value of serum 25-hydro- xyvitamin D: the higher, the median, and the lower groups. All subjects were asked for questionnaire and investigated for detailed physical examination, including age, height, weight, and history of disease and drug uses correlating to calcium and phosphorous metabolism. The bio-chemical indexes such as renal function, blood lipids and blood sugar were measured as well. The degree of arteriosclerosis was investigated with brachial-ankle pulse wave velocity (BaPWV) and ankle-brachial index (ABI). Results There were no significant differences between the three group among in general con- dition, cardiovascular risk factors, cardiovascular disease (CVD), neither in baPWV. However, ABI were significantly different (P=0.047) among the three groups, and significant negative (P=0.015) correlation was presented between ABI and serum 25-hydroxyvitamin concentration. Conclusions In elderly male persons, concentration of serum 25-hydro- xyvitamin is significantly negative correlated to ABI, but is not significantly correlated to baPWV. It is indicated that active vitamin D correlates to early arteriosclerosis.
