肢体缺血预适应增强脂联素对心功能的保护作用

摘要 目的:研究肢体缺血预适应对缺血大鼠心肌是否具有保护作用。方法:健康雄性SD大鼠,体重220~280 g,随机分为3组(n=10),假手术组(sham组)只穿线,左冠状动脉前降支不予处理;心肌缺血2 h组(MI2h组):心脏冠状动脉左前降支实施缺血2 h;肢体缺血预适应组(LIPC组):肢体缺血5 min,再灌注5 min,连续预适应3天,第4天实施缺血2 h。RM6240记录实验各组心功能指标,Elisa检测血清脂联素。结果:MI2h组大鼠血清中脂联素的含量、LVSP和±dp/dtmax与sham组比较均降低明显(P〈0.05);LIPC组大鼠血清中脂联素的含量、LVSP和±dp/dtmax较MI2h组升高(P〈0.05);与sham组相比,MI2h组LVEDP明显升高(P〈0.05),与MI2h相比LIPC组LVEDP数值降低显著(P〈0.05);血清脂联素水平与心收缩功能指标呈显著正相关。结论:脂联素与肢体缺血预适应作为正性调节因素,共同在心肌缺血损伤心肌病变的发生、发展中起作用。 Objective:To study the protection of limb ischemic preconditioning against myocardial ischemia(MI) injury in rats.Methods:SD rats(male,weight,220~280g).Rats were divided randomly into 3 groups(n=10).Group S(group in sham operated) were processed identically except the left coronary artery was not ligated;Group MI 2h(group in ischemia 2h) were subjected to occlusion of the left coronary artery anterior descending(LAD) followed by occlusion for 2h;Group LIPC(group in limb ischemic preconditioning) were subjected to ischemia on the left hind limb for 5 min in turn for 3 days.LAD was performed ischemia for 2h at the forth day.The function index of cardiac was recorded by RM6240.The titer of adiponectin was observed by Elisa.Results: Compared with Group S,the ADP level of blood plasma,LVSP and±dp/dtmax of Group MI2h decreased obviously(P0.05);Compared with Group MI2h,the ADP level of blood plasma,LVSP and ±dp/dtmax of Group LIPC increased significantly(P0.05);Compared with Group S,LVEDP of Group MI2h increased obviously(P0.05);Compared with Group MI2h,LVEDP of Group LIPC decreased significantly(P0.05).The level of serum adiponectin was correlated significant positively with heart systolic function parameters.Conclusions: As a positive regulated-factor,ADP and limb ischemic preconditioning worked on myocardial lesions of myocardial ischemia injury.
