摘要
目的了解本院产超广谱β-内酰胺酶(ESBLs)大肠埃希菌对氨基糖苷类的耐药情况及耐药基因的表达状况。方法用聚合酶链反应(PCR)法检测aac(3)-Ⅱ、aac(6′-)I、ant(2′′-)I、ant(3′′-)I、aac(3-)I等5种氨基糖苷类修饰酶(AMEs)基因。结果在临床分离的75株大肠埃希菌中共检出产ESBLs菌47株(62.67%);产ESBLs菌携带A-MEs基因检出情况,阳性率从高到低为:aac(3)-Ⅱ、aac(6′-)I、ant(2′′-)I、ant(3′′-)I、aac(3-)I。产ESBLs菌耐药情况从高到低依次为:链霉素、庆大霉素、妥布霉素、奈替米星、阿米卡星。结论大肠埃希菌产ESBLs率显著升高,耐药性增加;有些菌株同时携带两种或两种以上AMEs基因;ESBLs的产生可使大肠埃希菌对氨基糖苷类抗菌药物(AGs)的耐药情况加重。
Objective To study molecular epidemiology of the resistance to aminoglycosides(AGs) in Escherichia coli which produce extended spectrum β-lactamases(ESBLs).Methods Six kinds of AMEs of 75 strains E.coli.were detected by PCR.Results In 75strains of E.coli.,47(62.67%) ESBLs-producing strains were comfirmed.The Positive rate of Five kinds of AMEs were aac(3)-Ⅱ,aac(6′)-I,ant(2′′)-I,ant(3′′)-I,aac(3)-I,from high to low.Resistance to aminoglycosides were streptomycin,gentamicin,tobramycin,netilmicin,amikacin,,from high to low.Conclusion E.coli.resistance to AGs became more serious because of ESBLs producing.Some strains had two or more than two kinds of AMEs gene.
