米非司酮与去氧孕烯炔雌醇治疗围绝经期功能失调性子宫出血的效果比较

Curative Effect Comparison Between Mifepristone and Desogestrel-ethinylestradiol in Perimenopausal Dysfunctional Uterine Bleeding

目的评价米非司酮与去氧孕烯炔雌醇治疗围绝经期功能失调性子宫出血的疗效。方法将围绝经期功能失调性子宫出血患者随机分为米非司酮组(36例)和去氧孕烯炔雌醇组(34例),治疗后比较两组的疗效。结果两组患者治疗期后,月经周期恢复正常,经量减少,但组间差异不明显(P>0.05);治疗后血清FSH、LH、PRL、T变化不明显(P>0.05),E2和P显著低于治疗前(P<0.05);米非司酮组平均血红蛋白升高值、复发率和闭经发生率均高于去氧孕烯炔雌醇组(P<0.05)。结论两种药物用于治疗围绝经期功能失调性子宫出血效果良好,值得临床推广。

Objective To evaluate the clinical therapeutic effects of mifepristone versus desogestrel - ethinylestradiol in treatment of perimenopausal dysfunctional uterine bleeding. Methods Seventy patients with perimenopausal dysfunctional uterine bleeding were randomly divided into mifepristone group （n= 36） and desogestrel - ethinvlestradiol group （n= 34）. After the treatment, the curative effects of the two groups were compared. Results After the treatment, normal menstrual cycles restored and the amount of blood loss reduced, there was no significant difference between the two groups （P〉0.05）. E2 and progesterone levels decreased significantly in both groups （P 〈 0.05）, but follicle - stimulating hormone, luteinizing hormone, and testosterone levels did not change significantly （P 〉 0.05）. The incidence of amenorrhea, recurrence rate and the average increase value of hemoglobin in patients treated with mifepristone were all higher than those with desogestrel- ethinylestradiol （P 〈 0.05）. Conclusions Oral administration of mifepristone or desogestrel - ethinylestradiol is efficient and feasible for perim- enopausal dysfunctional uterine bleeding, which is worthy to be popularized in clinical practice.
