凝血指标在系统性红斑狼疮疾病中的临床应用

Clinical Application of Coagulation Indexes in Systemic Lupus Erythematosus

目的:检测系统性红斑狼疮(SLE)患者外周血中凝血指标及其相关指标来评价临床意义。方法:选用46例SLE患者为观察组,其中活动组26例,非活动组20例;40例正常人为对照组。测定血浆凝血酶原时间(PT),活化部分凝血活酶时间(APTT),血浆纤维蛋白原(FIB)和凝血酶时间(TT),血浆D-二聚体(D-Dimer,D-D)和纤维蛋白(原)降解产物(FDP),血浆抗凝血酶活性(AT:A);其他实验相关指标C反应蛋白(CRP),血沉(ESR)。结果:SLE组D-D及FDP明显高于对照组,PT及APTT比对照组明显缩短(均P<0.01),其他各指标均无统计学意义;SLE活动组ESR、CRP、D-D明显高于非活动组(均P<0.01),而FDP差别无统计学意义。结论:D-D及凝血指标能及时反映SLE患者体内凝血和纤溶系统失衡情况,对于SLE病程发展和控制有其重要的临床指导价值。

Objective Assess the clinical significance of coagulation and related indexes in patients with systemic lupus erythematosus. Method 46 patients with SLE（26 with active period and 20 with inactive period） and 40 normal subjects as control were studied. PT, AP3T,FIB,TT,AT：A,D-Dimer,FDP, CRP, ESR were detected. Results D-Dimer,FDP of SLE patients were much higher than those in the normal control, meanwhile PT and APTT showed shorter obviously. These differences which described above had statistical significance （P 〈 0.01 ）. Plasma D-Dimer, ESR and CRP levels of SLE patients with active period were much higher than those in inactive period. Conclusion Plasma D-Dimer and levels of coagulation indexes can reflect the imbalance of coagulation-fibrinolytic system in patients with SLE, furthermore they might play an important role in efficient prediction and controlling of happening of SLE.
