高尿酸血症与动脉粥样硬化关系的Meta分析

Systematic Evaluation on the Relationship between Hyperuricemia and Atheroselerosis

[目的]探讨高尿酸血症与动脉粥样硬化之间的关系,为两种疾病的预防控制和治疗提供参考依据。[方法]2010～2011年,运用Meta分析对国内外2005～2010年公开发表的研究高尿酸血症与动脉粥样硬化之间关系的文献进行综合分析。[结果]合计检索到78篇文献。其中血液指标方面51篇,有18篇纳入Meta分析,全部为病例-对照研究;基因方面27篇,有14篇纳入Meta分析,全部为病例-对照研究。血液指标中计量资料综合分析得出,高尿酸血症组总胆固醇(TC)、甘油三酯(TG)、动脉内膜-中层厚度(IMT)及动脉粥样硬化类疾病组尿酸(UA)均高于对照组(P<0.01),高密度脂蛋白胆固醇低于对照组(P<0.05);二分类资料综合分析得出,高尿酸血症组TC、TG以及动脉粥样硬化组UA都高于对照组(P<0.01);基因指标载脂蛋白E基因(ApoE)和亚甲基四氢叶酸还原酶基因(MTHFR)多态性与高尿酸血症和动脉粥样硬化类疾病关系密切。[结论]高尿酸血症是动脉粥样硬化的一个重要的危险因素,动脉粥样硬化也影响高尿酸血症的病情发展,并且两种疾病在基因多态性上有相同之处。

[Objective]To study the relationship between hyperuricemia and atheroselerosis,so as to provide the reference for the prevention and treatment.[Methods]Meta-analysis was used between 2010-2011 to synthetically evaluate the studies on relationship between hyperuricemia and atheroselerosis published in domestic and foreign journals between 2005-2010.[Results]A Total of 78 studies were searched.18 case-control studies were classified into the Meta-analysis from the 51 studies about blood parameters;14 case-control studies were classified into the Meta-analysis from the 27 studies about gene.Within the comprehensive analysis of measurement data of blood biochemical,the value of total cholesterol,triglycerides,artery intima-media thickness in the HUA and uric acid in the AS was higher than the CON（P0.01）,high-density lipoprotein in the HUA was lower than the CON（P0.05）;within the comprehensive analysis of dichotomous data,total cholesterol,triglycerides of the HUA and UA of the AS were higher than the CON（P0.01）;regard to the gene,the apolipoprote in E gene and methylenetetrahydrofolate reductase gene was contact with hyperuricemia and atheroselerosis closed.[Conclusion]Hyperuricemia is an important risk factor for atheroselerosis,and atheroselerosis can affect the development of hyperuricemia.
