妊娠期糖尿病患者血同型半胱氨酸水平及其相关性研究 被引量:27

Blood homocysteine level of the patients with gestational diabetes mellitus and correlational study

摘要 目的:通过检测妊娠期糖尿病患者确诊时及同期健康孕妇的血同型半胱氨酸(homocysteine,Hcy)、血糖、血脂水平,以了解妊娠期糖尿病患者血Hcy水平及相关因素。方法:选择2007年8月~2009年8月间门诊确诊的60例妊娠期糖尿病患者作为观察组,30例同期健康孕妇作为对照组。通过对血Hcy及相关指标(空腹血糖FPG、总胆固醇TC、甘油三酯TG)的测定,比较两组患者血Hcy水平,并分析血Hcy与相关指标的相关性。结果:与健康孕妇组比较,观察组患者血Hcy水平显著增高(17.67±3.45 vs 12.34±2.98,P<0.05)。观察组患者血FPG、TC、TG水平也明显高于对照组。增高的同型半胱氨酸与FPG、TC、TG具有明确的正相关关系。结论:妊娠期糖尿病患者血Hcy水平明显升高,与患者血糖、血脂水平密切相关。提示血Hcy水平的测定可作为妊娠期糖尿病患者的一个监测指标,且干预血糖、血脂等相关因素可能降低患者血Hcy水平,减轻妊娠期糖尿病患者大血管和微血管的损害。 Objective: To understand the blood homocysteine (HCY) level of the patients with gestational diabetes mellitus (GDM) and the related factors by detecting the levels of HCY, blood glucose, blood lipids in blood samples of the patients with GDM when they were diagnosed' definitely and the healthy pregnant women during the same period. Methods: Sixty patients with GDM who were diagnosed definitely in outpatient department of the hospital from August 2007 to August were selected as observation group, while thirty healthy pregnant women were selected as control group during the same period. The levels of blood HCY in the two groups were compared by detecting the levels of blood HCY and related indexes ( including fasting blood glucose, total cholesterol, and triglyceride), the correlation between blood HCY and related indexes was analyzed. Results: Compared with control group (12. 34 ± 2. 98), the blood HCY level in observation group increased significantly (17. 67 ±3.45) (P 〈0. 05) . The levels of fasting blood glucose, total cholesterol, and triglyceride in observation group were significantly higher than those in control group. There was a positive correlation between increased blood HCY level and fasting blood glucose, total cholesterol, and triglyceride. Conclusion : The blood level of HCY in the patients with GDM increases signif- icantly, which is associated with the levels of blood glucose and blood lipids closely, which indicates that blood HCY detection can be used as a monitoring index for the patients with GDM, and intervening the related factors, such as blood glucose and blood lipids, may reduce the level of HCY in blood and relieve the injuries of great vessels and capillaries among the patients with GDM.
