血清胱抑素C对急性心肌梗死存活患者近期预后的影响 被引量:2

THE PREDICTIVE VALUE OF CYSTATIN C FOR SHORT-TERM PROGNOSIS OF SURVIVAL PATIENTS WITH ACUTE MYOCARDIAL INFARCTION

摘要 目的评价血清半胱氨酸蛋白酶抑制物C(Cystatin C,Cys C)对急性心肌梗死存活患者近期预后的预测价值。方法 2010年6月—2011年2月收治急性心肌梗死患者168例,根据入院24h内血清Cys C水平,分为正常血清Cys C组82例和高血清Cys C组86例,观察2组患者Killip分级水平,2周内左室射血分数,3个月内心血管事件,探讨血清Cys C对急性心肌梗死存活患者近期预后的影响。结果高血清Cys C组在年龄、高血压病史、陈旧性心肌梗死病史、Killip分级、3个月内心血管事件均显著高于正常血清Cys C组(P<0.05),高血清Cys C组左室射血分数显著低于正常血清Cys C组(P<0.05)。血清Cys C是急性心肌梗死存活患者3个月内发生心血管事件的独立危险因素(OR=5.393,P=0.022)。结论血清Cys C是急性心肌梗死存活患者近期预后的预测因子。 Objective To evaluate the predictive value of Cystatin C ( Cys C) for short - term prognosis in survival patients with acute myocardial infarction. Methods One hundred and sixty - eight patients with acute myocardial infarction admitted from June 2010 to February 2011 were enrolled and divided into the normal Cys C group with 82 cases and the high Cys C group with 86 cases according to the level of Cys C. The level of killip classification and the left ventricular ejection fraction were observed in the two groups during hospitalization, the cardiac events were observed in three months. The effects of Cys C on prognosis of survival patients with acute myocardial infarction were evaluated. Results The mean age, the history of hypertension, the history of old myocardial infarction, the level of killip classification,and the cardiac events in three months in high Cys C group were higher than those in normal Cys C group ( P 〈 0.05). The left ventricular ejection fraction in high Cys C group was lower than that in the normal Cys C group ( P 〈 0.05 ). Multiple regression analysis indicated that the level of Cys C was an independent risk factor of cardiac events. Conclusion Cys C could predict short - term prognosis of survival patients with acute myocardial infarction.
