两种术式切除原发性翼状胬肉的疗效观察

Two Surgical Excision of Primary Pterygium Efficacy

目的：比较单纯胬肉切除术基础上分别联合自体角膜缘干细胞移植术和应用丝裂霉素C治疗原发性翼状胬肉的临床疗较。方法：将98例122只眼患者分为两组,在单纯胬肉切除术基础上分别采用角膜缘干细胞移植术（58例60只眼,简称干细胞移值组）和应用丝裂霉素C（50例62只眼,简称丝裂霉素组,）术后随访1~2年。结果：角膜上皮平均愈合时间：干细胞移植组为（4.0±0.8）d,丝裂霉素组为（7.3±1.7）d,两组差异有统计学意义（t=14.08,P〈0.05）。干细胞移植组2只眼复发,复发率为3.3%,丝裂霉素组14只复发眼,复发率为22.58%,两组差异有统计学意义（χ2=9.91,P〈0.05）。结论：单纯胬肉切除术,联合自体角膜干细胞移植术治疗原发性翼状胬肉角膜创面修复快,复发率低,是一种理想的手术,值得推广应用。

Objective：To compare the clinic effects of corneal limbal atem cell transplantion and simpie pterygium excision plus mitomycin Con primary pterygium. Methods： Ninty eight patients （122eyes）were duviede into two groups randomly . Corneal sfem cell transplant group included 48patients （60eys） , while comnon ptergium excision plus mito mycin C incluede 50patients （62eyes）. Postoperative follw - up time was 1 -- 2 years. Results： The average epithelial recovering time of corneal wound were 4.0 ± 0.8 days （stem cell group ） and 7.3 ± 1.7 days （mitomycin C group ） respectively. The differences of two groups were significant（t = 14.08, P 〈 0.05）. Two eyes reapsed in the stem cell transplant group（recurrence rate was 3.3 % ）, and 14 eyes rslapsed in the mitomycin C group （recurrence rate was 22.58 % ）, the differences was significant （Х^2 = 9.91, P 〈 0.05） . Conclusion： The corneal limbal stem cell tranplantion in an ideal surgical method on primary pterygium for the shorter time of recovering and lower rate of recurrence.
