输尿管镜下钬激光碎石术治疗输尿管结石86例 被引量:10

摘要 目的:探讨输尿管镜下钬激光碎石术治疗输尿管结石的临床疗效。方法:对86例采用输尿管镜钬激光碎石术治疗的输尿管结石患者的碎石率、排石率及并发症等进行统计分析。结果:单次碎石成功82例;3例结石上移至肾盂,置双J管后行体外冲击波碎石;1例因结石远端狭窄、扭曲,输尿管镜无法插入,改开放手术。平均手术时间(42.3±7.4)min,术后平均住院时间(2.5±1.7)d。术后随访1~3个月,结石排净率98.8%(85/86)。术中输尿管穿孔1例,术后发热6例。结论:输尿管镜下钬激光碎石术治疗输尿管结石是安全可靠的微创手术方法,是输尿管中下段结石首选的治疗方法。 Objective:To investigate the clinical effects of holmium laser lithotripsy through ureteropyeloscopy on patient with ureteral calculi.Methods:The data of 86 cases underwent holmium laser lithotripsy through ureteropyeloscopy were analysed retrospectively.Gralet rate,lithagogue rate and complication were statistically analysed.Results:Among 86 cases,gralet rate was 82 cases in a single procedure.Ureteral stone moved up into the pelvis in 3 cases,a double J tube was placed in the renal pelvis and extrocorporeal shock wave lithotripsy was used.A case with serious narrow distal ureters was conducted open operation.The mean operative time was(42.3±7.4) min and the mean hospital stay was(2.5±1.7) days.The duration of follow-up was 1 to 3 months for all patients.The stone free rate was 98.8%(85 of 86).Ureteral perforation and postoperative hyperpyrexia happened in 1 case and 6 cases respectively.Conclusions:The method of holmium laser lithotripsy through ureteropyeloscopy is safety,reliability and less invasion,so it is the best way to cure the middle and distal ureter calculi.
