BiPAP无创正压通气联合参麦注射液治疗矽肺COPD并呼吸衰竭疗效分析

BiPAP Noninvasive Positive Pressure Ventilation Combined Shenmai Injection in the Treatment of COPD With Respiratory Failure

目的观察BiPAP无创正压通气联合参麦注射液治疗矽肺COPD并呼吸衰竭的临床效果。方法对广西职业病防治研究院2007-01/2011-01收治的32例矽肺并慢性阻塞性肺疾病(COPD)及呼吸衰竭患者在常规治疗的同时,使用双水平气道正压通气BIPAP联合静滴参麦注射液治疗,在治疗前后,测定其动脉血气分析,并观察临床症状及治疗效果。结果与治疗前相比,ph值、PaO2、PaCO2、SaO2均有明显改善(P<0.01)。28例患者好转出院;2例患者因患多种慢性疾病合并肺部感染病情加重;2例患者病情加重行气管插管机械通气治疗,成功撤机好转出院。结论 BiPAP无创正压通气联合参麦注射液在矽肺COPD并呼吸衰竭治疗中有效、安全,明显降低了气管插管或气管切开以及相关的并发症,减少了病人的痛苦和医疗费用,同时在缩短疗程,提高疗效,提高患者的生活质量方面具有重要意义,参麦作为辅助治疗有一定的作用,值得临床推广。

Objective To observe the clinical effect of BiPAP noninvasive positive pressure ventilation combined shenmai injection in the treatment of COPD with respiratory failure.Methods From January,2007 to January,2011,32 cases of silicosis and chronic obstructive pulmonary disease（COPD） patients with respiratory failure received conventional treatment.At the same time,double level airway positive pressure ventilation combined static drops BiPAP shenmai injection was used in the treatment.Arterial blood gas was determined before and after treatment,with the clinical symptoms and treatment effect were observed.Results The pH value,PaO2,PaCO2 and SaO2 had improved obviously after the treatment（P 0.01）.Tweenty-eight patients got better and discharged;2 patients with chronic disease got worse because of multi-pulmonary infection;2 patients of exacerbation received tracheal intubation of mechanical entilation with successful weaning,got better and discharged.Conclusion BiPAP noninvasive positive pressure ventilation combined shenmai injection in the treatment of COPD with respiratory failure is effective and safe,could reduce the tracheal intubation or trachea cut and related complications effectively,reduce the patients＇ suffering and medical expense.In short-term treatment course,BiPAP can enhance the curative effect,improve the life quality of the patients.As auxiliary treatment,BiPAP has certain effect,and is worth clinical promotion.
