摘要
目的:对120例糖尿病患者颈动脉彩色多普勒超声(CDFI)检测指标和经颅多普勒超声(TCD)检测指标进行对比分析,讨论颈动脉CDFI各检测指标与TCD各检测指标之间的关系。为临床选择检测项目提供参考。方法:选取120例糖尿病患者作为研究对象,所有患者均同时完成颈动脉CDFI及TCD检查。首先将120例患者分为有斑块组和无斑块组,观察糖尿病患者颈动脉有无斑块与TCD各组检测指标之间的关系;再将其分为无斑块组、单侧斑块组、双侧斑块组进行对比分析,分析斑块数量与TCD各组测值之间的相关;并将斑块组分为硬斑组、软斑组和混合斑组3个小组,再对该组数据进行分析和比对,分析斑块性质与TCD各组测值之间的关系;最后对颈动脉(以右侧颈动脉为测量部位)内-中膜厚度(IMT)与TCD检测指标进行比对,分析IMT与TCD参数值间的关系。结果:与无斑块组相比,有斑块组的各条比对血管PI、RI、S/D值升高,差异有统计学意义(P<0.05),而Vp、Vd、Vm差异无统计学意义。斑块数量、斑块性质、IMT与TCD检测指标无相关性关系。结论:除有无颈动脉粥样硬化斑块外,其余颈动脉CDFI检测指标与TCD检测指标无相关性关系,因此联合应用颈动脉CDFI和TCD检测,才能较全面地反映脑血管和颈动脉结构及血流动力学状况,为临床诊治疾病提供参考。
Objective: To correlation and analysis of carotidartery color doppler flow imaging (CDFI) indices and transcranial dopplor ultrasound indices of 120 cases of diabetes. To analyze the correlation of two groups of indices and to provide the reference for clinical testing project. Methods: The clinical data of 120 cases of diabetes were included in the study and all of them received CDFI and TCD examination. The patients were divided into plaques group and no plaques group. The test was used to identify the correlation between the presence of carotid plaque and TCD indices. Then, the patients were also divided into no plaques group, unilateral plaques group, the bilateral plaques group, analyzed of the quantity and TCD each patch measured values of between related; And would be divided into hard spot group of plaque, soft spot group and mixed spot group three team, and the data was analyzed and compared, analyzed the nature and TCD patches of the relationship between the measured values; The last of the carotid artery (with the right carotid artery for monitoring site)- the film thickness in (IMT) and TCD examination indexes were compared, analyzed the relationship between the parameters in TCD and IMT. Results: The pulsatile index (PI), resistance index (RI) and ratio of peak systolic to least diastolic flow velocity (S/D) in patients with carotid plaque were significantly higher than those in patients without carotid plaque (P〈0.05); Vp, Vd, and Vm were not significantly different. The quantity/distribution of plaque, the quality of plaque and IMT were not correlated with TCD indices. Conclusion: No CDFI indices, except for the presence of carotid plaque, are correlated with TCD indices. Combine application of CDFI and TCD indices can better reflect the structure and hemudynamic status of cerebral vessels.
