加巴喷丁预给药对全身麻醉后导尿患者膀胱刺激症的影响

Influence of gabapendin premedication on irritative bladder symptoms in patients with urinary catheter after general anesthesia

目的探讨加巴喷丁预给药对全身麻醉患者留置导尿管后膀胱刺激症的影响。方法择期在全身麻醉下行开腹全子宫切除手术的患者80例,美国麻醉医师学会(ASA)分级Ⅰ～Ⅱ级,随机分为加巴喷丁组(G组)和对照组(C组)。G组在麻醉诱导前1h予加巴喷丁600mg口服,C组术前未给药。所有患者均于手术结束后留置16FFoley导尿管,球囊注射10mL0.9%氯化钠溶液,用胶布固定在耻骨弓上区域,并且没有任何牵引。观察并记录入麻醉恢复室即刻(0)、1、2、6h患者膀胱刺激症的发生情况及严重程度。结果 G组到达麻醉后监护室(PACU)0、1、2、6h的膀胱刺激症发生率均显著低于C组(P值均<0.05),两组间膀胱刺激症严重程度的差异亦均有统计学意义(P值均<0.05)。结论术前1h口服加巴喷丁600mg可以降低术后膀胱刺激症的发生率及严重程度。

Objective To investigate the effect of gabapendin premedication on irritative bladder symptoms in patients with urinary catheterization after general anesthesia.Methods Eighty American Society of Anesthesiology（ASA） grade Ⅰ-Ⅱ patients,treated by elective abdominal hysterectomy under general anesthesia,were randomly divided into gabapentin group and control group.Patients in gabapentin group were orally given 600 mg gabapentin 1 h before anesthesia whereas no administration in control group.Urinary catheterization was done with a 16F Foley catheter after surgery in all the patients,and its balloon was inflated with 10 mL 0.9% sodium chloride solution.The catheter was fixed in the suprapubic region with adhesive tape without traction.Irritative bladder symptoms were assessed 0 h（arrival in the PACU）,1 h,2 h,and 6 h postoperatively.Results The incidence rate of irritative bladder symptoms in gabapentin group was significantly lower than that in control group at each time point（all P0.05）.The difference in the severity of irritative bladder symptoms had statistical significance（all P0.05）.Conclusion Gabapentin（600 mg） administered orally 1 h before surgery can reduce the incidence rate and severity of catheter-related bladder discomfort.
