64层螺旋CT血管成像对夹层动脉瘤的诊断价值 被引量:6

摘要 目的探讨64层螺旋CT血管成像对主动脉夹层动脉瘤的诊断价值。方法回顾性分析25例经手术或临床确诊的夹层动脉瘤患者的64层螺旋CT表现,所有病例行平扫和增强扫描,并利用多平面重建法(MPR)、曲面重组法(CPR)、最大密度投影法(MIP)、容积成像法(VR)进行血管成像。结果平扫示主动脉管径增粗,腔内见低密度分隔影或内膜钙化高密度内移影,呈现蛋壳样及斑片状钙化。增强扫描示主动脉呈真假双腔影和剥离的内膜片呈低密度分隔线影,以及受累主动脉其分支和范围。二维图像(MPR和CPR)能够较好显示病变的范围、大小和病变细节,而三维重建图像(MIP,VR)可以更好地显示病变与周围空间的关系。结论 64层螺旋CT是诊断主动脉夹层的有效方法,结合影像后处理重建技术是早期确诊主动脉夹层的首选检查。
