腹腔镜辅助胃癌根治术50例临床分析

Clinical analysis of Laparoscopic-assisted radical gastrectomy 50 cases

目的探讨腹腔镜根治术治疗进展期胃癌的疗效及安全性。方法回顾性分析2010年5月至2011年5月临床接受腹腔镜下胃癌根治性切除术患者50例,其中10例根治性全胃切除术,10例根治性近端胃癌根治术,30例根治性远端胃癌根治术。结果失血量为术中(212±86)ml,清扫淋巴结(20.7±9.7)枚,术后肠道功能恢复时间为(2.6±1.2)d。术后住院时间(12±5)d,手术时间(252±31)min。全部50例成功完成腹腔镜胃癌根治手术。术后随访1～54个月,2例例带瘤生存,其他未发现肿瘤复发或转移,无死亡病例。结论腹腔镜辅助胃癌根治术安全、可行;严格遵守肿瘤的手术原则,腹腔镜辅助胃癌根治术能够保持肿瘤的根治性,同时能体现手术的微创性。

Objective To evaluate the efficacy and safety of laparoscopic radical in treatment of advanced gastric cancer. Methods A retrospective analysis of May 2010 - May 2011 in our hospital underwent laparoscopic resection of gastric cancer patients 50 cases, 10 cases radical total gastrecto- my, 10 patients with proximal gastric cancer with radical surgery, 30 cases of distal gastric cancer with radical surgery. Results The blood loss （surgery 212 ~ 86） ml, lymph node dissection （20.7 ＋_ 9. 7） pieces, after bowel function recovery time （2.6 -＋ 1.2） d. Postoperative hospital stay （12 -＋ 5） d, surgical time （252-＋ 31 ） min. Of laparoscopic gastric cancer surgery successful completion of all 50 cases. Postoperative follow - up 1 - 54 months, 2 patients with cases of survival, the other found no tumor recurrence or metastasis, no deaths. Conclusions Laparoscopic assisted radical gastrectomy safe and feasible ; strict compliance with the principles of cancer surgery, laparoscopic - assisted radical gastrectomy can maintain tumor cure, while minimally invasive surgery can be reflected.
