胃癌组织中端粒酶活性与基质金属蛋白酶-7表达的检测及其临床意义

The clinical significance of telomerase activity and MMP-7 expression in gastric carcinoma tissues

目的:检测胃癌组织中的端粒酶活性和基质金属蛋白酶-7(MMP-7)的表达,并探讨其临床意义。方法:在49例胃癌组织(转移30例,未转移19例)和20例癌前病变组织中,采用RT-PCR法检测MMP-7 mRNA,TRAP法检测端粒酶活性。结果:49例胃癌组织中,MMP-7 mRNA及端粒酶阳性表达者分别为28例和32例,其中转移组表达分别为21例和20例;20例癌前病变组织中MMP-7、端粒酶表达分别为4例和8例。胃癌组联合检测均阳性表达者19例,均阴性者8例;癌前病变组联合检测均阳性者仅3例,均阴性者11例。而MMP-7 mRNA表达在性别、年龄间差异无统计学意义(P>0.05);2组在病理分级中,Ⅰ、Ⅱ级的表达明显低于Ⅲ级(P<0.01);侵及浆膜层者明显高于未侵及者(P<0.01)。端粒酶在性别、年龄和病理分级以及是否侵及浆膜层间差异均无统计学意义(P>0.05)。结论:MMP-7、端粒酶与胃癌的发生、发展可能有关,高表达者胃癌转移的风险可能较高。联合检测较单一指标的检测率高。

Objective：To explore the clinical significance of MMP-7 mRNA expression and telomerase activity in the gastric cancer and gastric pre-cancerous tissue. Methods： MMP-7 mRNA expression was examined using RT-PCR technique and telomerase activity was detected using TRAP in 49 gastric cancer and 20 gastric pre-cancerous samples. Results： In 49 gastric cancer samples, the positive expression of MMP-7 mRNA and telomerase were 28 and 32 samples, and 21 and 20 samples in metastasis group respectively. In 20 samples with gastric pre-cancerous,the positive expression of MMP-7 mRNA and telomerase were 4 and 8 samples,respectively. Both MMP-7 mRNA and telomerase activity were showed the positive expression in 19 cases, while all negative were 8 cases in gastric cancer group. In gastric pre-cancerous group,both MMP-7 mRNA and telomerase activity were showed the positive expression in 3 cases, all negative were 11 cases. Gender and age were irrelevant with the expression of MMP-7. In the pathological grading of two groups, the level of I , II was significantly lower than that of I!1 （P 〈 0.01 ） and invades serosa layer was significantly higher than those of without （P 〈 0. O1 ）. Telomerase was irrelevant with gender, age, pathological classification and invades serosa layer（ P 〉 0.05 ）. Conclusions： MMP-7 and telomerase maybe relevant with the development of stomach cancer, the positive expression of MMP-7 and telomerase may suggest the metastasis of gastric cancer. The positive rate of joint detection is higher than single detection.
