右旋美托咪啶在小儿气管异物取出术中的应用 被引量:9

摘要 目的探讨右旋美托咪啶(Dexmedetomidine)应用于小儿气管异物取出术的效果,并与瑞芬太尼复合丙泊酚麻醉效果作比较。方法确诊为气道、支气管异物的患儿30例,随机平均分为两组:D组为右旋美托咪啶组,R组为瑞芬太尼复合丙泊酚组,观察患儿入室时、置入气管镜前、置入气管镜后HR、MAP、RR、SpO2,观察置入退出气管镜时、术中及术后喉痉挛、屏气、呛咳等情况。结果 D组及R组患儿用药后较入室时MAP、HR明显降低(P<0.05)。但均在正常范围;术中R组RR、SpO2值明显低于D组;术中D组发生喉痉挛、屏气、呛咳明显低于R组(P<0.05)。苏醒时间D组较R组约长,差异无统计学意义(P<0.05)。结论右旋美托咪啶用于小儿气管异物取出术较为安全可靠。 Objective To explore the effect of dexmedetomidine for removal of pedo-tracheal foreign body,which is compared to remifentanil combined to propofol.Methods 30 patiants were randomly divided into two groups with group D and group R.HR、MAP、RR、SpO2 were obverved when patiants entered operating room,when and after tracheascope was inserted.The times of laryngeal spasm,breathholding and bucking were,recorded intraoperatively,postoperatively and when tracheascope was inserted and extracted.Results MAP and HR of group D and R were lower obviously,but in normal range,after injection of dexmedetomidine or remifentanil combined to propofol.For group D,RR、SpO2 were higher and the times of laryngeal spasm,breathholding and bucking were lower and easier to resuscitate,individually,compared to that of group C.Conclusion Dexmedetomidine is safe and dependable for removal of pedo-tracheal foreign body.
