摘要 目的探讨磁共振(MRI)和氢质子磁共振波谱成像('H-MRSI)对颞叶癲痫(TLE)的定位诊断价值。方法选择15例经临床表现和脑电图诊断的颞叶癫痫患者和15例健康志愿者为研究对象,一次性行MRI检查及两侧海马结构的1H-MRSI检查。结果 MRI发现5例患者单侧海马硬化。1H-MRS可对8例患者进行定侧诊断,其中3例有双侧颞叶病变。将两种检查方法结合起来可对10例准确定位。结论1H-MRS对颞叶癫痫定侧敏感性高于MRI,MRI诊断海马硬化特异性较高。两者相结合可提高术前定位的准确率。 Objective To evaluate the value of MRI and 1H-MRSI in the lacalization diagnosis(LD)of temporal lobe epilepsy(TLE).Methods 15 selected patients with TLE verified by clinical manifestations and diagnosis of EEG and 15 healthy volunteers were studied.One-time conventional MRI and bilateral hippocampus 1H-MRSI inspection.Results 5 of these patients showed unilateral hippocampus sclerosis.1H-MRSI of hippocampal was positive in 8 cases,3 cases showing bilateral abnormalities,combination of the two methods can accurate positioning of locases.Conclusion 1H-MRS was more sensitive than MRI in lateralizing seizure origin,and bilateral lesions can be foung but MRI was more specific than 1H-MRS.Combination of MRS with MRI can improve the accuracy ratio of the localization diagnosis.
