糖尿病性黄斑水肿OCT和FFA形态学分类的对比分析 被引量:7

摘要 目的观察糖尿病性黄斑水肿(DME)患者光学相干断层扫描(optical coherence tomography,OCT)与眼底荧光血管造影(fundus fiuorescein angiography,FFA)的图像特征。方法对50例糖尿病性黄斑水肿患者进行OCT检查,其中48例同时行FFA检查,比较两种检查方法的结果。结果 OCT检查糖尿病性黄斑水肿患者黄斑中心凹平均厚度为(312.93±161.38)μm,黄斑水肿者占95.65%,包括海绵样水肿65只眼占70.65%,黄斑囊样水肿(cystoid macular edema,CME)40只眼占43.48%,神经上皮脱离者13只眼占14.13%,其中单纯海绵样水肿40只眼,单纯黄斑囊样水肿17只眼,单纯神经上皮脱离6只眼,海绵样水肿合并囊样水肿18只眼,海绵样水肿合并神经上皮脱离2只眼,三种水肿类型同时存在5只眼。OCT检查黄斑正常厚度及轻度水肿者FFA主要呈局限性水肿;中重度水肿者FFA主要呈弥漫性水肿和合并CME(黄斑区花瓣样荧光素积存)。OCT检查CME者为40只眼占43.48%,FFA检查CME者为25只眼占28.41%,二者CME检出率比较差异有统计学意义(χ2=4.43,P<0.05)。结论 OCT图像能量化显示视网膜增厚的程度,在检测CME方面优于FFA,而FFA在检测血管屏障方面比OCT敏感,两者的联合应用,有助于对糖尿病性黄斑水肿的诊断。
