非营养性吸吮在治疗早产儿胃食管反流中的疗效观察

Non-nutritive sucking in the treatment of gastroesophageal reflux in premature infant

目的:观察非营养性吸吮(Non-nutritive sucking,NNS)治疗早产儿胃食管反流(Gastroesophageal reflux,GER)的疗效,评估非营养性吸吮对减轻胃食管反流早产儿的呕吐症状,减少胃残留及促进胃排空的作用。方法:将早产儿胃食管反流患儿147例随机分为治疗组(74例)和对照组(73例),两组均根据体重、胎龄、出生日龄计算热卡,用同一种配方乳喂养,喂养不足的部分给予静脉营养补足,治疗组加用非营养性吸吮(NNS)。通过食管复方泛影葡胺(Compound meglumine diatrizoate)造影,观察患儿7 d及14 d后呕吐症状及食管复方泛影葡胺造影记录反流情况及胃残留率的变化。结果:治疗组7 d,14 d呕吐、胃残留发生率低于对照组,差异有统计学意义(P<0.05)。结论:NNS治疗早产儿胃食管反流疗效显著、安全、有效,可加快胃排空,有利于早产儿生后胃肠功能的发育,促进早产儿对喂养的耐受性,值得临床推广应用。

Objective To observe the non-nutritive sucking（Nonnutritive sueking,NNS） in the treatment of gastroesophageal reflux（GER） effect in premature infant,assessment of non-nutritive sucking on reducing premature infant＇ s vomiting,decreased gastric residual,and promoting the role of gastric emptying.Methods Select hospitalization＇ premature infant with gastroesophageal reflux were randomly divided into treatment group 147 patients（74 cases） and control group（73 cases）.both groups according to weight,gestational age,birth age calculated calories and using the same formula milk feeding,intravenous feeding less nutritional supplement some of the treatment group with non-nutritive sucking（NNS）. Through the esophagus diatrizoate compound（compound meglumine diatrizoate） contrast,observed in children with vomiting after 7 d and 14 d and esophageal imaging compound diatrizoate records regurgitation and gastric residual rate of change.Results The treatment group 7 d,14 d,vomiting,gastric emptying rate than the control group,the difference was statistically significant（P〈0.05）.Conclusion Pretmature infant NNS effect is significant gastroesophageal reflux,safe and effective,can speed up gastric emptying, premature children benefit the development of gastrointestinal function after birth,premature children to promote tolerance of feeding,is worthy of clinical application.
