彩色多普勒超声评价烟雾病患者颞浅动脉血流动力学的研究 被引量:1

摘要 目的探讨烟雾病患者颞浅动脉的血流动力学表现,为显微外科手术治疗烟雾病提供相关依据。方法应用彩色多普勒超声检测22例烟雾病患者(A组),18例动脉硬化患者(B组)及20例正常健康成人(C组)的双侧颞浅动脉起始段、顶支和额支的血流动力学参数,并进行统计学分析。结果 A组颞浅动脉的起始段、顶支和额支的内径、收缩期峰值流速(PSV)、舒张末期流速(EDV)及阻力指数(RI)与C组比较差异无统计学意义;但额支内径较B组狭窄(P<0.05),余参数与B组比较差异无统计学意义;B组颞浅动脉的起始段PSV较C组升高,其起始段其余参数以及其顶支、额支的各项血流参数与C组比较差异无统计学意义。结论烟雾病患者的颞浅动脉在管径、血流动力学参数方面与健康成人比较差异不明显,受外周动脉粥样硬化的影响较小。此可为显微外科借助颞浅动脉作为供血动脉治疗烟雾病提供相关依据。 Objective To explore the hemodynamics of superficial temporal artery in patients with moyamoya disease, and to provide basis for microsurgery of moyamoya disease. Methods Twenty-two patients with moyamoya disease( group A), 18 patients with arteriosclerosis (group B) and 20 healthy subjects (group C) were enrolled in this study, their initial segment, parietal branch, frontal branch of superficial temporal artery were examined by color Doppler ultrasound, then the hemodynamic parameters were analyzed by statistics. Results There was no significant difference in the internal diameter of initial segment, parietal branch, frontal branch of superficial temporal artery, peak systolic velocity ( PSV), end diastolic velocity (EDV) and resistant index(RI) between group A and group C. The internal diameter of frontal branch in group A was narrower than that in group B. PSV of superficial temporal artery initial segment in group B was higher than that in group C, while there was no significant difference of other parameters between group B and group C. Conclusion Superficial temporal artery has no significant difference of internal diameter and hemodynamic parameters between patients with moyamoya disease and normal adults, which is little influenced by peripheral artherosclerosis. This provides a valuable basis for using superficial temporal artery as feeding artery for microsurgery of moyamoya disease.
