地佐辛超前镇痛用于腹腔镜全子宫切除的临床研究

Clinical research on preemptive analgesia with Dezocine intotal laparoscopic hysterectomy

目的探讨地佐辛超前镇痛用于腹腔镜全子宫切除术的临床效果及安全性。方法选取80例行腹腔镜全子宫切除的患者，随机分成实验组（地佐辛组，40例1和对照组f芬太尼组，40例），比较两组患者麻醉诱导及气管插管时的心血管反应、术中麻醉药用量、苏醒时间、拔管时间、拔管后5min潮气量、PETCO，（呼气末二氧化碳）和拔管后1、2、4、8、16、24h视觉模拟疼痛评分（VAS）。结果实验组患者在麻醉诱导期的心血管反应明显小于对NN（P〈0．05），实验组术中丙泊酚和瑞芬太尼的用量明显少于对照组（P〈0．05），两组患者清醒及拔管时间、拔管后5rain的潮气量及PETCO．差异无显著性（p〉0．05），术后各时段疼痛评分对照组均明显高于实验组（P〈0．05）。结论地佐辛超前镇痛用于腹腔镜全子宫切除能平稳、有效、安全地维持较长效。

Objective To study the clinical effects and safety of preemptive analgesia with Dezocine in total laparoscopic hysterectomy （TLH）. Methods 80 cases of TLH were chosen, and randomly divide into 2 groups, including 40 cases of the experiment group （Dezocine group） and control group （Fentanyl group） respectively. The experiment group was given 0.2 mg/kg of Dezocine before anesthesia induction. We compared the cardiovascular response of tracheal intuhation, the dosage of anesthetic during the operation, as well as the recovery time, the extubation time, the 5-min tidal volume and PETCO2 after the extubation, and the l h, 2h, 4h, 8h, 16h, 24 h VAS scores after the extubation of 2 groups. Results The fluctuation of cardiovascular system in control group was obviously higher than that in the experiment group （P 〈 0.05）. Anaesthetic used during the operation in the experimental group was less than that in control group （P〉 0. 05）. The recovery time and the extubation time has no difference （P 〉 0.05）, as well as the 5-rain Tidal volume and PETCO2 after the extubation in 2 groups. The VAS scores after the extubation showed significant differences （P 〉 0.05）. Conclusions Dezocine can provide a perfect long-acting, steady, effective and save analgesic effect on TLH.
