摘要
目的了解持续中心负压吸引在预防乳腺癌改良根治术后皮下积液的作用。方法将临床符合标准的乳腺癌术后患者随机分为对照组和实验组。对照组术后常规连续单管或双管接一次性负压瓶负压引流及弹力绷带加压包扎3~5天。实验组利用持续中心负压吸引,负压维持在-0.01--0.03Mpa(-100--225mmHg)之间持续负压吸引,连续3~5天,比较两组患者皮瓣区皮下积液及皮瓣坏死发生睛况。结果实验组术后皮下积液2例,对照组9例,实验组术后皮下积液发生率明显低于对照组(P〈O.051实验组术后皮瓣坏死0例,对照组6例,实验组术后皮瓣坏死发生率明显低于对照组(P〈0.05)。结论持续中心负压吸引可用于预防乳腺癌改良根治及根治手术后皮下积液和皮瓣坏死的发生,并促进伤口愈合,值得在临床上推广应用。
Objective To study the function of continuous center attract negative pressure in preventing subcutaneous hydrops after radical mastectomy in breast cancer. Methods The patients accorded with the clinically standard after radical mastectomy were randomly divided into control group and experiment group, after radical mastectomy, the control group patients were tied up for 3 N 5 days by using normal regulations continuous single tube or double tube connecting a negative pressure, or with a bottle negative pressure to lead to flow and the elasticity bandage. The experiment group made use of continuous center attract negative pressure, pressure maintained at -0.01 N -0.03 Mpas(-100 ~ -225 mmHg) keeping on negative pressure, continuously for 3 ~ 5 days, we observed and compared the incidence rate of subcu- taneous hydrops and flap necrosis, and recorded a related data to have statistical analysis. Results The subcutaneous hydrops in the experiment group was 2 cases (8%), while control group occurred 9 cases (35%). The incidence rate of subcutaneous hydrops in the experiment group was obviously lower than that of control group(P 〈 O.05).No skin flap necrosis was noted in the experiment group (0%),while 6 cases found in control group(23%). The skin flap necrosis incidence rate in the experiment group was lower than that of control group with statistical significant difference (P 〈 0.05). Conclusions Continuous center attract negative pressure can be used in radical mastectomy to prevent the incidence rate of subcutaneous hydrops and flap necrosis, helping the wound to heal. Therefore it is worth to be applied in clinic.
