摘要 目的评价内窥镜下经x-tube腰椎板切除减压,髓核摘除术的初步临床效果。方法自2010年1月共收治13例腰椎间盘突出患者,其中L4/5节段7例,L5/S1节段6例,均行内窥镜下经x-tube腰椎板切除减压,髓核摘除术,按改良Macnab评分标准评价临床效果,同时记录手术时间,失血量及并发症。结果所有病例均得到随访,随访时间3~12个月,平均6个月。术后功能评定按改良Macnab评分分级:优10例,良3例,优良率100%。未发现手术并发症。手术时间45~105 min,平均60 min,失血量10~95 mL,平均30 mL。结论METRx内窥镜辅助X-Tube下治疗腰椎间盘突出症,具有微创直视化、管道化特点,效果满意;应严格把握手术适应症,术中仔细操作,避免手术并发症发生。 Objective To investigate the effect of decompressive laminectomy and discectomy under METRx endoscope-assisted X-tube in the treatment of lumbar disc herniation. Methods 13 patients with lumbar disc herniation underwent decompressive laminectomy and discectomy using x- tube operation system, including lumbar disc herniation of L4/5 in 7 cases, L5/S1 in 6. According to Macnab scale, the surgery effect, operative time, intraoperative blood loss and complications were evaluated. Results All operation were completed successfully, and the patients were fellowed up with an average of 6 months (range from 3 to 12 ). Macnab sacle: excellent in 10 cases, good in 3 cases at the third month follow-up after operation. Operative time were 60 rain (45-105 min), intraoperative blood loss were 30 mL (10-95 mL). No complications were found. Conclusion With minimally invasive effect and direct vision, the decompressive laminectomy and discectomy using x-tube operation system has advantage for treatment of lumbar disc herniation. It is a key for successful operation to strict indication and standard operating.
