大型医院与社区卫生服务中心建立对口转诊预约机制实践探讨

Discussion on setting up mechanism of making an appointment of partner referral between large hospital and community health centre

首都医科大学宣武医院通过与社区卫生服务中心建立对口转诊预约机制,签署协议书,完善组织结构,制定规章制度和岗位职责,建立例会制度,本着实名制转诊预约、转诊预约优先、开放全部号源和分时段就诊的原则,规范转诊预约流程,逐步推进社区患者转诊预约就诊模式。提出应积极探索大型医院与社区卫生服务中心对口转诊预约的长效机制,发挥大型医院在社区转诊预约中慢性病防控的作用。

Appointment referral mode which involves establishing partner appointment referral mechanism, signing agreement, improving organization structure, setting regulation and position guide, setting regular meeting system has been established between affiliated Xuanwu Hospital Capital Medical University and community health center. Under principles of real name appointment, referral first, sharing all sources, the referral process has been standard optimized and the mode has been gradually extended. Long term partner referral mechanism between large hospital and community health center should be improved to develop the functions of large hospital in chronic disease prevention.
