大骨瓣减压联合选择性亚低温治疗脑外伤分析

Effect of large trauma craniot combining with selective hypothermia in therapy of brain injury

目的探讨脑外伤患者采用大骨瓣减压手术并联合选择性头部亚低温疗法,观察两者联合作用的疗效和GOS的评分情况。方法 82例脑外伤患者,试验组43例,采用大骨辧减压手术与选择性亚低温联合治疗;对照组39例,采用大骨辧减压手术与常温治疗,观察并比较两组颅内压、GOS预后有效率。结果试验组在1、2和3 d的颅内压值分别是(19.6±2.4)、(20.2±2.1)、(17.1±2.0)mm Hg,较对照组明显下降(P<0.05);对照组和试验组的半年预后有效率(GOS评分)分别为25.64%和46.51%(P<0.05),且试验组的植物状态病死率比对照组低(P<0.05)。结论对脑外伤患者采用大骨瓣减压手术联合选择性头部局部亚低温疗法疗效显著。

Objective To study in head-injured patients with large bone disc decompression, and joint selective head and low temperature therapy, observe the effect of both united and GOS ratings. Methods Selected 82 cases of brain injury patients as a study object in our hospital from January 2007 to June 2011. For large trauma craniot and selective hypothermia combination therapy had 43 cases. We observed and comparative analysis of the two groups GOS better prognosis, intracranial pressure rate, and analysis of the difference between the two groups curative effect. Results The patients in 1 day, 2 and 3 days of intracranial pressure value were （19.6 ±2.4）, （20.2 ±2.1）, （17.1 ±2.0） mm Hg, which declined obviously （P 〈 0.05）; In the control group and the group a half a year better prognosis rate （GOS score） were 25.64% and 46.51% respectively; The total effective rate of the former was obviously higher than those in the control group （P 〈 0.05）. Conclusion In head-injured patients with large bone disc decompression jointing selective head and low temperature treatments have a good effect.
